THIS IS A SOURCES SOUGHT NOTICE (SSN) ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. DISCLAIMER This SSN is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this SSN that is marked as proprietary will be handled accordingly. IAW FAR Part 13.106, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Interested sources are solely responsible for all expenses associated with responding to this notice. SOURCES SOUGHT DESCRIPTION The purpose of this notice is to gain knowledge of potential qualified sources and their size classification relative to the NAICS code 812210 Funeral Homes and Funeral Services. The small business size standard is $8.0M. Responses to this notice will be used by the Government to make appropriate acquisition decisions. After review of the responses to this notice, a solicitation announcement may be published. Responses to this notice are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office 22, is seeking capable sources that are classified and registered in www.vip.vetbiz.gov as either Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Businesses (VOSB) that can provide mortuary and burial services in support of VA San Diego Healthcare System (VASDHS). The anticipated period of performance is one 12-months Base Period and four 12-months Option Periods). The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. Interested and capable businesses of providing the services described in this notice, shall provide the information indicated below. Response to this notice should include business name, address, point of contact, size of business pursuant to the following questions:  Identification of business size status, Cage Code and SAM ID. Please indicate the size standard and representation of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.). Indication of which North American Industry Classification System (NAICS) code(s) your business usually performs under Government contracts. Proof of experience and capability to provide the service indicated on this notice. If you intend to sub-contract any work, what portion of the total cost will be self-performed/will be performed by your organization? Please provide estimated detailed percentage breakdowns related to sub-contracted work and completion of job. Does your business have an FSS contract with GSA or the VA or are you a contract holder for any other federal contract? If so, please provide the contract number. General pricing of your service is encouraged. Pricing will be used for the purpose of market research only. It will not be used to evaluate for any type of award. Please review the attached Performance Work Statement (PWS) and provide feedback or suggestions. If none, please reply as N/A. Responses to this notice shall be submitted via email to Ms. Monica Ann Reed, Contract Specialist, monica.reed@va.gov. Telephone responses will not be accepted. Responses must be received no later than 4:00 PM PST on 25 April 2022. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this notice. Responses to this notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. PERFORMANCE WORK STATEMENT -DRAFT SCHEDULE OF SUPPLIES/SERVICES AND PRICES/COSTS The Contractor shall provide mortuary services consistent with the requirements of this solicitation and Veterans Health Administration Handbook 1601B.04. No minimum quantities of orders are guaranteed. Services include but is not limited to the removal of the body from VA Facility, transportation, body preparation (embalming), clothing, securing all necessary permits, ensuring a United States flag accompanies the casket to place of burial and inspection. Services will be required are on an occasional, repetitive, and rotational basis. This is a non-exclusive contractual agreement for participating mortuaries to perform mortuary, burial, and transportation services for unclaimed Veteran remains and to provide transportation of the deceased remains to and from the San Diego VA Medical Center for the purpose of autopsy. The burial allowance may not be paid to the extent that it was paid by the deceased s employer or by a State agency or political subdivision of a State. This agreement is dependent on your willingness to bill at the burial allowance (including technical & professional charges) allowed by VA regulation, which shall constitute payment in full. Cost associated with transporting remains is additional to the burial services and plot allowance. VA will pay for the actual cost of transporting the Veteran s body to the place of burial in state, as provided in 38 U.S.C. 2303(a)(1)(B) and 38 CFR 3.1605(b). Transportation allowance covers: (1) The transportation cost of shipment of the body by common carrier or by hearse from the VA facility or place of death to the funeral home and place of burial; (2) Any charges for an outside shipping box; and (3) The charges for securing all necessary permits for removal or shipment of the body. SERVICES: Decedent Affairs Office shall contact the Mortuary Company to arrange pick-up of deceased Veteran. Response time is to be within 24 hours after call is placed or specified time. Veteran s pick-up location (unless directed differently): 3350 La Jolla Village Drive San Diego CA 92126. Burial services will include the following specifications: 1. Preparation of the body: Embalming and refrigeration. 2. Clothing: The deceased Veteran s body will be dressed in suitable and appropriate attire. Veterans shall be dressed for burial. 3. Casket: The casket, at a minimum, must be constructed from thick, strong particleboard and must be of sufficient strength to support the weight of an adult human body. Cardboard or press paper or similar materials are not acceptable. 4. Permits: Mortuary Company will secure all necessary permits to provide service. Reimbursement for such permits will be included in transportation charges. 5. United States Flag: The Mortuary will ensure that a U.S. Flag, provided by the Funeral Director at the VA, accompanies the casket to the place of burial. 6. Transportation: The body will be transported by hearse or van from the VA facility to the funeral home and to the place of burial. Veteran buried at the Miramar Veterans Cemetery. 7. Inspection: The Chief, Health Administration Service, or designated representative will be responsible for medical inspection of the mortuary services performed and inspection of the merchandise furnished. This designee will also be responsible for certifying receipt on receiving report and forwarding for 10-2068a to Acquisition and Materials Management service to ensure payment for services performed. The VA representative is the Decedent Affairs clerk. The Decedent Affairs clerk will be responsible for inspection of the mortuary services performed. The Decedent Affairs clerk also has the authority to certify that the services and correct and necessary items have been furnished. CONTRACTOR VEHICLES: All contractor vehicles utilized in contract will be insured and maintain a current state vehicle registration. Contractor is responsible for ensuring their staff retains a valid State Driver s license. Contracted staff shall not have any traffic violations on his/her driving record. LICENSES: Provide to this office not later than 7-days after award, a current copy of your Certificate and License issued to your establishment. Faxed copies will be accepted. ORDERING: A contract number shall be issued and will apply to this agreement and must be cited on all invoices and correspondence. Ordering officers to be determined at time of award. Upon receipt of Authorization Form 10-2065, the VA generates a purchase order number authorizing services. Once services are complete, contractor (Mortuary Company) submits billing invoice to the VA. All submitted invoice should reference the Contract Number and associated Purchase Order, both provided by VA. Please note the Contract Number bracketed with this agreement must be cited on all invoices and correspondences for payment. Decedent Affair s staff, Health Benefits & Enrollment Manager, and COR assist the Contracting Officer with monitoring the contractor s performance, certification of invoices, and other administrative aspects. Please be reminded that their authority is limited and that all changes to the contract remain the sole responsibility of the Contracting Officer. RETENTION/DESTRUCTION/RETURN/SECURITY: Management for Retention, destruction and return of VA information of decedents shall be adhered to as follows: Retention (Archive) of VA documents: minimum 1-Year. Destruction after: 75 years (copy of destruction certification required and shall be provided to the VA). Return of VA documents, upon request after completion of job: 5-days. Death certificates, Vital record information with PII must be secured under lock and key to prevent identity theft. Documents must also be secured when transporting and not left in an open vehicle. TERMINATION OF MORTUARY SERVICE CONTRACTS: Should the Government or Contractor choose to cancel services at any time during the contract period, a written notice is required, and to be provided 90-days in advance of desired cancellation date. Termination of contract is effective 90 days from date stated on written notification. EXCEPTION FOR USES AND DISCLOSURES OF VA INFORMATION: As authorized herein for performance of the contract, the contractor may use and disclose VA information only in two other situations as directed below: The contractor must refer all requests for production demands or inquiries about VA information or information systems to the Contracting Officer and/or Contracting Officer Representative (COR). If the contractor/subcontractor is in receipt of a court order or other requests for the above-mentioned information, that contractor/subcontractor shall immediately refer such court orders or other requests to the VA Contracting Officer and COR for response.
