Description
Performance Work Statement Cable Service provided for: Corporal Michael J. Crescenz VA Medical Center and Community Living Center (CLC) Purpose: To provide strong cable services to the Main VA Hospital and Community Living Center (CLC). The Contractor shall furnish all labor, equipment, supplies, and materials needed to complete the installation of cable services to the number of outlets/floors identified at the Main Hospital and CLC. This service will require access to local, fixed package of channels and access to all Philadelphia Sports. Location: Main VA Hospital located at 3900 Woodland Ave Philadelphia, PA 19104 and CLC located at 3461 Civic Center Blvd Philadelphia, PA 19104. 2. Installation will need to be coordinated with the project Contracting Officer Representative (COR) at facilities and phased for minimum impact to our veterans. 3. Qualifications: Shall be authorized to provide materials, installation, and full-time support for uninterrupted services at both facilities. Invoices and Payment Invoices will be submitted monthly in-arrears to the Government office designated in the solicitation and include the original. Required Work Provide and install equipment, supplies, and materials to support strong signal cable services to 425(+/-) outlet and floors identified at the Main VA Hospital and CLC. Vendor will maintain all associated equipment including but not limited to distribution cables, amplifiers, pedestals, lock boxes, equipment, and appurtenant devices. Test the incoming signals and make the necessary adjustments for tv s located in all patient rooms, waiting rooms, conference rooms and emergency operations centers throughout both facilities. The VA requests a variance of 5 units prior to any renegotiations in monthly service frees. The VA will notify the vendor of any increase or decrease in the number of TV s. Schedule 1. Contractor must provide an on-site response time capability of 2 hours to respond to cable/internet problems or outages. An immediate telephone response capability is also required, using a discreet phone number to which the VA may report problems or outages, and with a maximum wait time of 3 minutes. 2. There may be times that work will have to be performed off-hours due to emergency or electrical shutdown requirements. Vendor must have the ability to provide services in these events. E. Other Criteria: 1. Contractor shall provide electronic copy of any updated channel lineup for distribution to Patient rooms and nursing stations. F. Period of Performance/Term/Warranty 1. The term of the service contract shall be for 1 year with 4 possible option years. The warranty on any newly installed equipment shall be provided for at least a 3-year manufacturer s warranty on all parts and labor. The warranty shall include all travel and shipping costs associated with any warranty repairs. Period of Performances (PoP) dates: Base period: 06/01/2022 thru 05/31/2023 Option Year 1: 06/01/2023 thru 05/31/2024 Option Year 2: 06/01/2024 thru 05/31/2025 G. Security/Privacy: 1. Contractor will be required to comply with physical security guidelines by checking in with the Facilities Electric Shop each time they come on-site to perform contracted services. 2. Contractor will not have access to any VA-sensitive information under this contract. 3. Contractor must be always escorted by appropriate VA personnel while performing work in sensitive areas. H. COR/ Alternate POC: 1. COR: Lisa Ziegler, FMS/Activations Coordinator 215-823-5800 ext. 205315: point of Contact (POC) for all invoices and coordination of initial installation. 2. Alternate POC: Frank Howard Electrician/FMS at 215-823-5800 ext. 205170: POC for day-to-day for services or any interrupted services that will require onsite response.
