Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Responses to this sources sought notice must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential businesses interested and capable of providing the services described below. After review of the responses to this Sources Sought, a solicitation announcement may be published. Responses to this Sources Sought synopsis are NOT considered adequate responses for a solicitation announcement. The Veteran s Integrated Service Network 22 is seeking a contractor to provide daily patient specific sterile preparation for parenteral nutrition therapy and deliver to the VA Loma Linda Pharmacy daily as per patient specific orders. This requires a contractor that is registered to prepare compounded (prepared from raw ingredients) IVs for administration in a hospital that meet FDA and USP requirements for sterility and processing. The prospective contractor must be able to meet the following requirements: 1. Have trained, and accredited personnel to create custom TPN formulas per VA Loma Linda Pharmacy guidelines. 2. Recognize that TPN formulas may change daily and be able to accommodate changing requirements. Should the item not be in stock, the vendor will make a reasonably good faith effort to obtain the prescription from another pharmacy or supplier to adequately fill the order. If the order cannot be filled completely, VA Loma Linda will be notified at least three (3) hours prior to delivery. 3. Provide TPN undamaged and transported under proper storage requirements. In the event TPN is damaged or was not transported under the proper storage conditions, the vendor shall produce a replacement immediately at no cost to V.A. Loma Linda Health Care System. 4. Provide all necessary materials, labor, supervision, and management to: (A) coordinate delivery: (B) load and unload; (C) and deliver to VA Loma Linda Pharmacy Inpatient Pharmacy. 5. Follow all 3-in-1 TPN guidelines (Amino Acids, Dextrose & lipid in one bag). 6. Contracted pharmacist will be on-call until 9:30 pm to remake any leaky bags, etc. VA Loma Linda Pharmacy must inspect bags upon delivery (check the order, press on the bags lightly to check for any leaks, etc.) and inform contractor immediately to cover rare occasions when remake needs to be done. 7. Mixing of TPN solutions shall be in accordance with the American Society of Hospital Pharmacies (ASHP), The Joint Commission (TJC) guidelines, Current USP 797/800 standards, American Society for Parenteral and Enteral Nutrient (ASPEN) and any revisions or new USP standards identified. 8. Contractor shall provide macronutrient and micronutrient components for TPN compounding and will include additives such as vitamins, electrolytes, Insulin, and other additional additives as ordered per prescriptions. 9. VA LL Nutrition Support Team will provide prescription order by fax, phone, encrypted email or contractor proprietary web-based ordering system by 1230hrs. The contractor will notify VA Loma Linda Nutrition Support team to confirm the receipt of each order. 10. Contractor shall provide same day delivery by 1730hrs, Monday thru Sunday. This includes all Federal Holidays. 11. Contractor shall provide quality assurance data as required by JCAHO on a quarterly basis. 12. Contractor shall provide a customer service representative available 24 hours a day, 7 days a week. 13. Pharmacy will need on average three TPN's per day, but the quantity could increase or decrease depending on patient census, not to exceed 2.5 liters. If you are interested and are capable of providing the required service(s) please provide the following information. (1) Company Name, Address, Point of Contact, SAM Unique Entity ID, DUNS number, and Cage Code. (2) Size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc. (3) Is your company considered small under the NAICS code 325412 Pharmaceutical Preparation Manufacturing? (4) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with any other federal contract? If so, please provide the contract type and number. (5) If you are an FSS GSA/NAC or other federal contract holder, are the referenced items/solutions available on your schedule/contract? (6) Please provide general pricing for your products/solutions for market research purposes. (7) Please submit a capability statement that addresses qualifications and verifies ability to provide required services. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought announcement. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Businesses. However, if there are insufficient Service Disabled Veteran Owned Small Business responses, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. Responses to this notice shall be submitted via email to peter.kim33@va.gov . Telephone responses will not be accepted. Responses must be received no later than 3/31/2022 10:00 AM PDT. After review of the responses to this sources sought announcement, a solicitation may be published on the SAM.gov or GSA eBuy websites. Responses to this sources sought announcement will not be considered adequate responses to the solicitation. All interested offerors must respond to the solicitation in addition to responding to this sources sought announcement. Prospective bidders must be authorized re-sellers in order to participate in the solicitation process. Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov. Additionally, all Service Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at www.vip.vetbiz.gov.
