The VA is currently conducting market research to locate qualified, experienced, and interested potential sources. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this Sources Sought announcement. NCO 1 Contracting is seeking interested sources for a future solicitation and procurement. At this time, no solicitation exists. This is for information and planning purposes. BACKGROUND: VISN 1 Healthcare System (New England) has a requirement to provide Basic Life Support (BLS) and Advanced Life Support (ALS) Ambulance Services to its beneficiaries., including medical services and personnel, for patients of the Connecticut VA Medical Centers. The Connecticut VA Medical Center includes the two (2) major medical centers and nine (9) community-based outpatient clinics (CBOCs) listed below: VA Medical Facilities Locations: West Haven VAMC Newington VAMC 950 Campbell Ave 555 Willard Ave West Haven, CT 06516 Newington, CT 06111 Community Based Outpatient Clinics (CBOC): Danbury Clinic Errera Clinic John J. McGuirk VA Outpatient Clinic 7 Germantown Road 114 Orange Avenue 6 Shaw s Cove Suite 2b West Haven, CT 06516 Suite 104 Danbury, CT 06810 New London CT, 06320 Newington VA Clinic Orange VA Clinic Stamford VA Clinic 555 Willard Avenue 200 Edison Road 1275 Summer Street Newington, CT 06111 Orange, CT 06477 Suite 102 Stamford, CT 06905 Waterbury VA Clinic Willimantic VA Clinic Winsted VA Clinic 95 Scovill Street 1320 Main Street 115 Spencer Street Waterbury, CT 06706 Willimantic, CT 06226 Winsted, CT 06098 OBJECTIVES: VISN 1 Healthcare System is looking for small business concerns, especially Service Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB) contractors that can perform ALS and BLS Ambulance Services, non-profit and large business concerns are also welcomed to respond to this notice. The current primary NAICS code of this requirement is 621910 (Ambulance Services), which has a size standard of 16.5 million. 4. SUBMITTAL INFORMATION: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response as detailed below. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill the requirements above. If this information is not provided, your firm cannot be considered a viable source for this procurement. Interested parties shall furnish the following minimum information: Company name and address ensure your company is registered in the System for Award Management (SAM) at www.sam.gov. Dunn and Bradstreet (DUNS) number. Provide SAM unique entity identifier (UEI) (if available). Company point of contact name, telephone number, and email address. Evidence that you are a small business (if applicable): Small Business ensure you are listed as small business in SAM, reps and certifications. Provide the NAICS codes you are listed as a small business under. CVE certified SDVOSB or VOSB (or in the process of certification) ensure you are certified in the CVE veteran database at www.vetbiz.va.gov/. Provide evidence of certification. If your company holds a Federal Supply Schedule (FSS) Contract, list the GSA Contract Number and SINs. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills/required licenses your company possesses to perform services in the geographic areas described above. Evidence of existing capability to perform the required transportation services with all necessary resources in place upon the execution of the awarded contract. 6. Past Experience: Brief summary of the company s history relative to similar requirements in size and scope. Provide the information on projects of similar size and scope for which the responder was a prime or subcontractor including size of fleet and numbers of employees. Provide contract number the work was performed under, period of performance and one professional reference with contact information.
