Description
THIS IS A SOURCES SOUGHT/REQUEST FOR INFORMATION (RFI) issued in accordance with Federal Acquisition Regulation Part 10, Market Research. It is for information, planning and market research purposes only and shall not be construed as either a solicitation or obligation on the part of the Oklahoma Army National Guard (OKARNG). The OKARNG welcomes responses from all interested parties. The OKARNG has a general requirement for a certified instructor to provide a course in �Human Factors Analysis and Classification System (HFACS) and Human Factors Intervention Matrix (HFIX)�, in accordance with the AR 385-10, DA PAM 385-10, and the 385-40 regulations. The instructor is responsible for providing all training materials (training slides, videos, handouts, books, and teaching aids) necessary to facilitate the training successfully. At the end of the course the instructor must have the authority to issue a federally recognized certification certificate to the student.� The course is intended for the OKARNG�s Additional Duty Safety Officers (ADSO) in human error analysis while conducting accident/incident investigation and reporting. The OKARNG has 25 students to be instructed in how to analyze and prevent errors associated with quality and safety events in the OKARNG. The classroom will be provided by the OKARNG for the dates 9-10 August 2022 in Norman, OK. The Army National Guard (ARNG) �Certified Accident Investigator Program (CAIP)� requires individuals to complete the following courses in order to be assigned/appointed as a certified ARNG investigator: Human Factors Analysis and Classification System & Human Factors Intervention Matrix, the National Transportation Safety Board (NTSB) Helicopter Accident Investigation, �NTSB Aircraft Accident Investigation, ARNG Accident Investigation and Analysis Course, TSI/FAA � Basic Aircraft Accident Investigation, and ARNG Accident Investigation Exercise (Boneyard Course). The students are to submit their course completion certificates along with an application to be recognized as a Certified Accident Investigator (CAI) to the ARNG safety office. All certificates received by the course instructors must be recognized by the ARNG CAI program. SUBMISSION REQUIREMENTS:�Interested parties are invited to submit a response to this notification by 2:00 p.m. CST on or before 31 March 2022, to the attention of Harvetta Taylor, email address harvetta.r.taylor.civ@army.mil .� All questions and industry feedback are asked to be submitted in writing to the Primary Point of Contract and will be addressed accordingly. Responses should be no more than 5 pages and include the following information:�Organization Name;�Contact Name, e-mail address and phone number;�Organizations DUNS number;�Business size status for the applicable NAICS code;�Federal Government Contract vehicle;�A statement of capability to perform details of the requirement;�Industry feedback including responses, questions, and comments;�Interested parties are instructed to provide only the data requested.
