Department of Veterans Affairs Network Contracting Office 16 - HOUSTON In Support of the Southeast Louisiana Veterans Health Care System (SLVHCS) REQUEST FOR INFORMATION 36C25622Q0516 BACKGROUND: Ceribell Rapid Response EEG Headbands & Recorder SLVHCS has a requirement for items listed below. Manufacturer- (Ceribell). Qty Item # Description/Part Number* 20/EA C152 Ceribell EEG Headband, Adult 2/EA C100 Ceribell EEG Recorder Statement of Work Ceribell Rapid Response EEG Headbands & Recorder Southeast Louisiana Veterans Health Care System New Orleans, LA Purpose The overall purpose is to provide a Ceribell Rapid Response EEG machine at Southeast Louisiana Veterans Health Care System (SLVHCS), New Orleans. Scope 2.1 The Contractor shall provide, transport, test and provide training on all listed equipment. All products must meet all salient characteristics defined in this section. 2.2 All equipment must meet manufacturers and VA specifications. 2.3 The Contractor shall furnish all supplies, equipment, facilities and services required for delivery of the supplies and equipment. 2.4 The Contractor is responsible for any missing parts and components not included in order to complete full delivery. Salient Characteristics Ceribell Rapid Response EEG portable machine, qty: 1 EEG Headband, qty: 20 8 channel setup and reading Brian stethooscope function EEG Portal EEG recorder portable, qty: 2 Micro-USB cable 180 mm x 80mm x 28mm, qty: 1 Ceribell EEG recorder software or equivalent Power adapter input 100-240 V ac output 5v dc 2.4 Ghz ieee 802.11 b/gn wifi interface DELIVERY: Contractor shall deliver all equipment to: Southeast Louisiana Veterans Health Care System (SLVHCS) New Orleans, VA Loading Dock, 2400 Canal St., New Orleans LA 70119 Inspection and Acceptance Delivery of Equipment Deliver materials to job in manufacturer's original sealed containers with brand name marked thereon. Package to prevent damage or deterioration during shipment, handling, and storage. Maintain protective covering in place and in good repair until removal is necessary. Store products in dry condition inside enclosed facilities. 5. DELIVERABLES 5.1 Operation and Maintenance Manuals 5.2 Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to COR upon delivery. 6. WARRANTY 8.1 Warranty shall be in compliance with manufacture warranty. REQUESTED RESPONSES: The intent of this Request for Information is to establish sources to define the procurement strategy (e.g. set-aside, sole source, unrestricted) for a solicitation that VA intends to post soon. Interested contractors are requested to respond in accordance with the following: Please respond to this RFI if you can provide the exact Brand Name name products listed in section above. In the response, please cite your business size status. If you have an existing GSA or VA, Federal Supply Schedule contract, please include the contract details in your response. DISTRIBUTORS: You must provide proof that you are an authorized distributor. VA does not accept grey market items. SDVOSB/VOSBs responding to this requirement, must be registered in Vetbiz https://www.vetbiz.va.gov. Please note that VA is particularly interested in determining the availability of Small Business Manufacturers. If your company is a small business manufacturer of potentially equivalent items, please respond to this RFI. Vendors are requested to submit estimated market research pricing with their responses. The estimated pricing will be considered when determining the procurement strategy for the future solicitation. (e.g. if CO determines that capable small businesses cannot provide fair and reasonable pricing, then the solicitation will not be set-aside). Please note that if no responses to this notice are received, from either authorized distributors of the cited brand name nor from manufacturers marketing a potentially equivalent brand, then this action will be sole sourced to the manufacturer cited in the Background section above.
