*****Please submit quote in accordance�with the attached SF1449***** STATEMENT OF WORK TITLE OF CONTRACT:� �Veterans Health Administration Warehouse 8k- 10K Forklift Rental SCOPE OF WORK: Veterans Health Administration has a requirement to rent one 8k -10K forklift. �BACKGROUND: OEM has a requirement to rent one (1) 8k � 10k forklift. By renting this unit, the logistics staff will be able to complete the necessary training on the unloading, set up, maintaining, take down and loading of the unit. OBJECTIVES: Local readiness and pricing was gathered to determine what equipment is currently available and cost of a unit for use.� The requirements are: 8k - 10000 lb. capacity sit down diesel forklift Reaching height required is a maximum of 144� Mast can be two or three staged wide view Length of forks must be 48� at a minimum Side shift and fork positioner option required Tires will be all terrain Back-up alarm and flashing beacon required �Side and rear-view mirrors �Front and rear flood lights The location of the servicing company needs to be within 35 miles of the facility to ensure prompt service is available in the event of equipment failure. DELIVERY - Upon acceptance of the contract the contractor will be required to deliver the forklift to 510 Butler Avenue, Martinsburg, WV 25405. Pick Up- Upon completion of the contract the contractor will be required to pick up the forklift at 510 Butler Avenue, Martinsburg, WV 25405. WARRANTY � The contractor will provide complete coverage for repair and preventative maintenance during the period of the contract. WRITTEN DOCUMENTATION � The contractor will provide a comprehensive owner�s manual and laminated quick operational guide for the unit. *****Please submit quote in accordance with the attached SF1449*****
