No formal solicitation document exists at this present time. This is not a solicitation announcement. The purpose of this Sources Sought notice is for market research to gain knowledge of potential vendors qualified under NAICS code 332322 (Sheet Metal Work Manufacturing) PSC 5410 (Prefabricated and Portable Buildings) NAIC business size standard of 500 employees for the Martinsburg VAMC. After review of the responses to this RFI, a solicitation announcement may or may not be published in GSA EBuy or Contract Opportunities. REQUESTED INFORMATION If you are interested and capable, and can provide the required supplies/service, please provide the requested information as indicated below: Responses to the 8 questions below for this Sources Sought notice shall be on a company letterhead including the company name, address, point of contact, DUNS number, size of business and include the requested literature. STATEMENT OF NEED The CBOC in Winchester VA for an audiology booth for the Martinsburg VA Medical Center s Winchester Virginia Community Based Outpatient Clinic (CBOC). An audiology booth is required for standard of care to provide the space to conduct hearing tests on our Veteran patients. Please review the specifications below and respond to all 8 questions. Please provide product literature/suggested drawing and specs, detailing what is included with the audiology booth. Must state manufacturer name on papers. Please provide literature regarding the original and extended warranties of brand or equal product. Please state whether this booth is customizable and provide product literature (see specifications below). Provide a copy authorized distributor letter if you are not the manufacturer. Is this item priced on contract with GSA? If the answer to question 5 is yes, please provide the contract type, contract number, category(s), sin, and source code on GSA. Is the above listed NAIC and PSC business size status under Vendor s SAM/VIP and GSA contract? What is the expected delivery date for the requested quantity?
