- THIS REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. The submission of pricing, capabilities for planning purposes, and other market information is highly encouraged and allowed under this RFI In Accordance With (IAW) FAR Part 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a RFI/sources sought only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339114 (size standard of 750 Employees). Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 15 Network Contracting Office, is seeking sources that can provide the following: Name brand or equal: A-dec 500B Basic dental equipment package with installation Three (3) Dental Operatory Chairs (A-dec 500 Basic Dental Chairs) Ultra-Thin flexible back rest, measuring no less than 1.25 thick on seamless upholstery providing operator access to the oral cavity providing patient comfort and support. Headrest, standard dual articulating with gliding cushion and quick release adjustment lever. 2-position armrest, mounted mid-thigh position, design articulates forward for patient access. Wear resistant, durable design for comfort and ease of asepsis. Lift range, 13.5 -31.5 (Note: measurements are made from the floor to the bottom of the seat cushion,) Chair swivels 60 degrees (30 degrees each side of center) Automatic Chair-Stop, to reduce risk of injury or damage to equipment when sensors detect obstruction. Lock-Out feature to disable chair movement when handpiece is in use. Integrated power supply of at least 300 watts to provide electrical requirements of chair-mounted dental light and ancillary products. Must have a minimum UL rated 500lb. (227 KG) maximum patient load. Self-diagnosing circuit board, that is easily programmable, with lift activation. Bi-directional hydraulic motor pump Cast iron baseplate, should be powder coat painted and sealed with an elastomeric polymer finish to prevent rust and scratch damage to the floor. Three (3) Traditional Style Mounted Delivery System with Touchpad Integrated four position handpiece control block, is capable of operating up to four active handpiece positions and is constructed of plated lead free brass (per section 14. 17 of the Safe Drinking Water Act) Each handpiece position must have an individual drive air and water adjustment. Removable water cartridge valves must allow for quick and easy servicing of block. A single sliding locking tab should secure the handpiece tubing to the control block requiring no tools and allow easy access to connect or disconnect tubing. Waterflow system needs to eliminate dead ends that can harbor stagnant water. Led Coolant Indicator Must have ability to customize placement, with at least six positions on the control head, to allow ability to integrate almost any device. Removable Tray Holder must be stackable and customizable. Left/Right side convertible without the use of tools. Chair Lockout Stop Feature to prevent the accidental movement of the chair when a handpiece or instrument is in use. Silicone Handpiece Tubing must contain anti-microbial AlphaSan and quick-disconnect. Must be secured to the control block. Do not want vinyl tubing. A 2.0 Liter, Quick-Connect water bottle designed with an interior pickup tube to reduce the chance of cross-contamination. The System Flush Toggle must have a flow-through design that eliminates stagnant water and flushes all tubing simultaneously. All Interior Water Lines used throughout the delivery system must contain anti-microbial AlphaSan to protect against microbial colonization on interior and exterior surfaces. The Control Head needs to provide ample space to integrate up to six clinical devices. The Flex Arm, to enable easy routing of cable and tubing. USB Extension Cable is provided in delivery system umbilical. Handpiece Holders are independently adjustable a full 45 degrees to allow precise positioning of instruments. Three (3) Dental Assistants Stool (A-dec (522)) Seat armature should have at least three layers of flexible seating; cushion foam, comfort-flex armature, and flex frame. Adjustable seat tilt feature for enhanced ergonomic positioning of about 4 degrees forward and 4 degrees backward tilt. Distributes weight from seat to feet. Stool is rated for at least 350lb. Three (3) Operator s (Doctors) Stool (A-dec (521)) Seat armature with at minimum three layers of flexible seating; cushion foam, comfort-flex armature, and flex frame. Stool is rated for at least 350lb. Three (3) Portable Wheeled Cart with Casters (A-dec Oregon Cart Locking- (5744 Locking)) Cabinet shells and related components parts are at least 5/8 thick, and at least a 45# industrial grade particle board. Door fronts covered with high pressure V32 laminate (HPDL). Locks need to be available. Top work surface slides side to side to expose a 7 deep storage well. Must have at least four storage drawers that lock open and have aluminum handles. Three (3) Mini LED During Light (A-dec (54.0291.00)) Installation- Technician set up and installation required for all equipment to: Kansas City VA Medical Center 4801 Linwood Blvd Kansas City, MO 64128 The information identified above are intended to be descriptive, not restrictive, and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications, or brand name equal. If so, please provide the contract number. Please submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement described in the attached SOW.
