Description

Project Title: Design-Build Interventional Imaging Surgical Suite in Building 1 Place of Performance: Veterans Affairs Central California Health Care System, 2615 E. Clinton Ave, Fresno CA Description: Contractor shall provide all supervision, labor, materials, services, tools, equipment, and transportation�for design and construction to upgrade OR #4 into an Interventional Imaging Surgical Suite in accordance solicitation,�Scope of Work and applicable attachments. IAW VAAR 836.204, Disclosure of the magnitude of construction projects, the magnitude of construction for this project�is between $2,000,000.00 and $5,000,000.00. Note #1 to OFFERORS: This RFP is being issued for Full and Open Competition. Note #2 to OFFERORS: This project will be awarded IAW FAR 36.3 - Two-Phase Design-Build Selection Procedures. Note #3 to OFFERORS: Bid Bonds, Payment Bonds and Performance Bonds shall be required under this solicitation and any�resultant contract. **Amendment 0001 extends the Phase One proposal due date and provides partial responses to RFIs. **Amendment 0002 extends the Phase One proposal due date and provides for an additional Site Visit. **Amendment 0003 extends the Phase One proposal due date, provides the SOW (Rev.1) and provides responses and supporting documents for RFIs.
