Contractor and Vendor questions regarding updates for the COVID Safety Protocols are directed to review online Safer Federal Workforce Task Force Guidance issued on September 24, 2021. The William S Middleton VA Medical Center ENT Department is requesting new specialized exam room chairs. They are requesting (Brand Name or Equal) (3) Maxi 4000 Exam Chairs due to the special salient characteristics of these chairs. These chairs are needed to maintain the full safety and access of all VA patients. The currently used chairs turn, but do not spin from the base, entire chair is unlocked and moves when needing to turn. Also, when Veterans exit the current chair, the chair tips forward and increases risk for a fall forward. Specifically, the Maxi 4000 Exam Chairs must have the following characteristics to meet the Governments Need: Trendelenburg Position Capability Has a minimum lift capacity of 400 pounds Turning capability from solid base with an automatic lock function Has an adjustable headrest Has exam light mounted from the back of chair Arm rests that lay flat when patient is leaning back in chair &/or lying down. Power reclines and lift feature Programmable memory settings At least 345-degree chair rotation with automatic lock Dual mounted chair controls Auto position return Footrest and arm rests lift for easy access for patients to enter and exit chair Lift height maximum of at least 38 Lift height minimum of 22 Seat width at least 22 Overall length of chair no more than 80 Overall width of chair no more than 40 (17 total salient characteristics needed) Needed items: (Three) MAXI 4000 ENT Exam Chairs; Color: Graphite (Three) SOLARLITE WITH ROTARY SWITCH IEC PLUG SHORT CORD FOR S4000 Place of Performance: William S.
