- THIS REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. The submission of pricing, capabilities for planning purposes, and other market information is highly encouraged and allowed under this RFI In Accordance With (IAW) FAR Part 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a RFI/sources sought only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 922160. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 15 Network Contracting Office, is seeking sources that can provide the following: Fire Extinguisher Inspections and Certification ST Louis VA Medical Centers DESCRIPTION OF SERVICES: The contractor must ensure all service technicians inspecting equipment at the VASTLHCS be in possession of the required certifications, in accordance with National Fire Prevention Association (NFPA) 10 standards for Portable Fire Extinguishers. The annual maintenance of portable fire extinguishers shall be performed in accordance with the manufacturers established operating standards applicable to NFPA 10 requirements. The contractor shall perform the annual inspections and maintenance in January. The maintenance shall include the 6-year test and/or hydrotest of all fire extinguishers at VASTLHCS when due or as alternate the cost of a new replacement extinguisher, whichever is less costly. Requests for clarification or additional information concerning scope or specifications shall be directed to the Contracting Officers Representative (COR). PERFORMANCE PERMIT: The Contractor shall make available to services required under this Statement of Work (SOW) commencing with the effective date of award, unless otherwise directed by the Contract Office (CO) and shall provide the required services until the date of contract expiration. The contract shall include a base year and four renewable, non-guaranteed, option years. SERVICES TO BE PROVIDED The contractor shall provide the current inventory at each location and conduct the annual inspections and maintenance for all potable fire extinguishers at the VASTLHCS at the following locations. The maintenance shall include hydrotesting and/or the 6-year testing or replacement, if due for that extinguisher. CBOCs: Manchester Avenue VA Clinic: 4974 Manchester Ave., St. Louis, MO 63110 Washington Avenue VA Clinic: 2727 Washington Ave., St. Louis, MO 63103 St. Clair County VA Clinic: 1190 Fortune Blvd., Shiloh, IL 62269 St. Charles County VA Clinic: 844 Waterbury Falls Dr., O Fallon, MO 63368 St. Louis County VA Clinic: 6854 Parker Rd., Florissant, MO 63033 Franklin County VA Clinic: 1627 A. Roy Dr., Washington, MO 63090 Olive Street VA Clinic: 3615 Olive St., St. Louis, MO 63108 Jefferson Avenue VA Clinic (Hope Recover Center): 515 N. Jefferson Ave., St. Louis, MO 63103 Vet Centers: St. Louis Veterans Center: 287 N. Lindbergh Ave., Creve Coeur, MO 63141 East STL Veterans Center: 227 West Point Dr., Swansea, IL 62226 Other Leased Spaces: R&D Admin: Big Brother/Big Sister: 501 N. Grand Blvd., St. Louis, MO 63103 Public School Retirement Bldg. (Blue Bldg.): 3641 Olive St., St. Louis, MO 63108 Note: The CBOCs, Vet Centers, and other leased space s locations are subject to change. The COR will provide an updated list of the locations of these spaces to the contractor for every option year. Additional Sites: The COR may add additional sites at any point during the performance period of the contract. The following services shall be provided for both VASTLHCS Divisions, CBOCs, Vet Centers, and other leased spaces. These services shall be done in accordance with NFPA 10. Annual Inventory: The contractor shall identify all portable fire extinguishers by the VA inventory sheets provided in Attachment 2. This information shall include the physical locations, fire extinguisher serviceability and maintenance status. This item is to be accomplished in January during the annual inspection and maintenance. Annual Inspection and Maintenance: The contractor shall inspect each fire extinguisher at the locations listed above. Each fire extinguisher must have an annual inspection performed to include new tags. The annual inspection shall include, at a minimum, the following items: Location in designated place. No obstruction to access or visibility. Operating instructions on nameplate are legible and facing outward. Safety seals and tamper indicators are not broken or missing. Fullness determined by weighing or hefting. Examination for obvious physical damage, corrosion, leakage, or clogged nozzle. Pressure gauge reading or indicator in the operable range or position and tag replacement. Leave prior year tag in place on all bottles. Hang new tag for the current year on all bottles and date (month/year) and initial for January. While doing the inspection the contractor shall identify the extinguishers that are due for maintenance. If an extinguish is due for maintenance, the contractor shall conduct either the hydrotest or the 6-year test. The 6-year test or hydrotest shall be conducted in accordance with NFPA 10 Standards. The contractor may opt to replace the extinguisher with a new extinguisher in leu of testing, if that is more cost effective. Note: When extinguisher is taken for maintenance the contractor is responsible for providing a loaner extinguisher for the extinguisher type taken. Exhaust Hood Inspection: The contractor shall conduct the semi-annual (every 6 months) exhaust hood inspection in accordance with NFPA 96, Standard for Ventilation Control and Fire Protection of Commercial Cooking Operation. The exhaust hood inspection shall be at the following location. Jefferson Avenue VA Clinic (Hope Recover Center): 515 N. Jefferson Ave., St. Louis, MO 63103. The COR may add additional sites at any point during the performance period of the contract. Note: The Hope Recover Center location is subject to change. The COR will provide an updated location to the contractor for every option year. SPECIAL WORK REQUIREMENTS The contractor must ensure all service technicians inspecting equipment included in this SOW shall be in possession of the required certifications per NFPA 10 standards. Identification: The Contractor s personnel shall wear either a company shirt or hat bearing the company logo. Vehicles: All Contractor s vehicles will be locked, and keys removed while performing service on the VA property. This is intended to protect the Contractor s property and provide for the safety of the VA Medical Center s personnel. Vehicle(s) shall bear the company logo. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Training and Experience: Contractor representatives who perform services under this contract must be competent, experienced, and qualified to perform such services listed herein. The Contractor shall submit proof of appropriate training and experience for vendor personnel performing services. Safety Requirements: In the performance of this contract, the Contractor shall take such safety precautions as the COR or his/her designee may determine to be reasonably necessary to protect the lives and health of VA patients, visitors, staff, and the general public. The COR or his/her designee shall notify the Contractor of any noncompliance with the foregoing provisions as well as the recommended corrective action. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served in person on the Contractor or his/her representative at the site of work, or telephonically to Contractor s designated representative, shall be deemed sufficient for the purpose of the previously mentioned. If the Contractor fails or refuses to comply promptly to satisfactorily abate the hazardous condition or situation, the COR may issue an order stopping all or any part of the work and hold the Contractor in material breach of this contract. Damage: The contractor shall take all necessary precautions to prevent damage to any government property. The Contractor shall repair any damage to the premises caused by the Contractor, its employees, or the agents. The Contractor shall hold harmless and indemnify the Government from and against any and all losses or damages resulting from the Contractor s installation, maintenance, service, removal, or operation of the equipment or any other equipment of the Contractor s except loss or damage arising from negligent or intentional act or omission of the Government. Restrictions: Smoking is prohibited on VA grounds. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States (Federal) District Court, not a local district state, or municipal court. Personal Identification Verification (PIV): The Contractors staff performing duties at either Division shall be fully PIV registered and shall be completed 45 days after contract award. Except for initiating registration with the PIV Sponsor, personnel conducting business pursuant to this contract shall not enter VA owned or leased property without a current and valid VA issued badge. Personnel may also be subject to inspection of their personal effects, including vehicle, or any other container, whilst on VA owned or leased property upon request from VA Police. Contractor shall coordinate with COR for PIV badging. SUBMITTALS The Contractor shall, without additional cost to the VA, provide and maintain all licenses and permits for operational personnel for proper inspection and maintenance of fire extinguishers in accordance with all applicable Federal, State, municipal, and local regulations [e.g., NFPA 10). If appropriate licenses are not maintained in accordance with Federal, State, and local requirements, the VA may terminate the contract effective upon discovery. The VA reserves the right to halt work if the COR determines work is being done in an unsafe/unhealthy manner or that could harm the environment. The VA St. Louis Healthcare System shall not incur additional costs if work is halted for good cause. Schedule: The Contractor shall submit, 30-days after award, the planned schedule. Inventory Reports: Upon the completion of work, the contractor shall submit a completed inventory report for each location to the COR. Note: The contractor shall provide the inventory reports in an excel document. Upon completion of the annual inspection, the contractor shall provide to the COR the number of fire extinguishers inspected at each location and a list of extinguishers due for maintenance (i.e., 6-year test or hydrotest). Upon completion of each semi-annual exhaust hood inspection the contractor shall notify the COR or his/her delegate. The contractor shall also provide a detailed list of any necessary maintenance and the cost associated with such maintenance. If for any reason any deliverable cannot be delivered within the scheduled time frame, the contractor is required to explain why in writing to the COR, including a firm commitment of when the work shall be completed. This notice to the COR shall cite the reasons for the delay, and the impact on the overall project. The COR will then review the facts and issue a response, in accordance with applicable regulations. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. Cost incurred by the Contractor through the actions of parities other than the CO, or those specifically delegated to the COR, shall be borne by the Contractor. If you are interested, and are capable of providing the sought out supplies/services, please provide the requested information as well as the information indicated below. Response to this notice should include company name, address, point of contact, size of business pursuant to the following questions: Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.)? If you re a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? Does your company have an FSS contract with GSA, VA NAC, NASA SEWP, or any other federal contract, that can be utilized to procure the requirement listed above? If so, please provide the contract number. Please submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement described in the attached SOW.
