Description
Department of Veterans Affairs, Albany Stratton VA Medical Center, Network Contracting Office 2, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide a comprehensive vehicle for full preventive maintenance, repair and software update services for six Alcon Medical Lasers in the surgical OR s of the New York Harbor Healthcare System s Manhattan and Brooklyn Campus , SERVICES REQUIRED: Contractor shall provide on-site scheduled preventive maintenance (specified by OEM frequency), repair and software upgrade for two CONSTELLATION LXT, one LENSX LASER 100-0001, and three CENTURION VISION SYSTEMS New York Harbor Healthcare Systems Manhattan Campus located at 423 E 23rd St, New York, NY 10010 and Brooklyn Campus located at 800 Poly Pl, Brooklyn, NY 11209 during the term of November 16, 2021 through November 15,2022 with provision for four (4) Option Years. Contractor shall provide services for CONSTELLATION LXT, one LENSX LASER 100-0001, and three CENTURION VISION systems. INTERESTED & CAPABLE RESPONSES: NCO 2 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services. Please provide all of the following in your response: Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address), GSA Contract number if applicable, and DUNS number. Please also include the North American Industry Classification System (NAICS) Code for which you propose to provide the service under for verification of your size standard. A Capability Statement which provides detailed, specific information that demonstrates understanding of the tasks required and capability to perform the tasks as stated in the attached Statement of Work. Failure to tailor capability statement to this specific requirement may result in your company being determined non-responsive. This Special Notice of Intent to Sole Source is for planning purposes ONLY. The results of this notice may assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted for this notice. STATEMENT OF WORK Alcon Medical Laser Full-Service BACKGROUND: The Department of Veterans Affairs, New York Harbor Healthcare System, is currently seeking full preventive maintenance (PM) services for Alcon Medical Lasers in the surgical OR s of the Manhattan and Brooklyn Campus . These units are used in ophthalmic surgeries or medical procedures. In order to keep these units in optimal functioning order and to reduce malfunctioning, causing a lapse in functionality, and preserve patient safety, the facility requires full annual preventive maintenance, repair, and software update service, as well as on-call support and on-site repair. SCOPE OF WORK: The contractor shall provide all labor, tools, equipment, travel, parts, and services necessary to accomplish the preventive maintenance, repair, software updates, on-call support, and on-site repair for the Alcon Medical Lasers listed. Full coverage of the machines shall include, but not be limited to: On-site annual planned maintenance On-site annual recertification On-site repairs and service All repair/replacement parts Unlimited labor for all services On-call telephone support Case support Clinical trainings for staff Shipping charges All peripherals The contractor shall perform annual on-site scheduled preventive maintenance services in accordance with OEM specifications. Preventive maintenance shall include, but not be limited to full technical inspection, all parts, services, manuals, tools, and software updates required to complete the full preventive maintenance services. Upon completion of the preventive maintenance, contractor shall appropriately tag the equipment with the applicable labels used by each facility, if required. The contractor shall submit a schedule of preventive maintenance and a corresponding checklist describing in detail the maintenance procedures for the inspection of each piece of equipment within ten (10) days of contract award. The contractor shall utilize these procedures and checklists with worksheet originals indicating the work performed and actual values obtained (as applicable) provided to the COR at the completion of the PM. All covered equipment will have PM service performed as specified by the OEM frequency. The contractor shall maintain the equipment in accordance with OEM specifications, and shall include but not be limited to calibration, cleaning, oiling, adjustment, replacement parts, maintenance of equipment, and software updates. Contractor shall respond with a phone call to the COR and their designee within one (1) hour after receipt of telephoned notification. Any unscheduled on-site repairs shall be completed within 24 hours of beginning of work if solution is not available via telephone. C. OTHER RELATED SERVICES (NOT INCLUDED IN THE STATEMENT OF WORK: Any services not expressly stated in this statement of work are not authorized. Services such as inspection, Original Equipment Manufacturer (OEM) required maintenance, or other maintenance and repair vital to the continued functioning of the systems may be added only after written modification from the contracting officer, unless they are inclusive to the requested services. No other person, including the COR or any other VA personnel are authorized to request additional work be performed by the contractor. Any additional services performed that are not authorized by the contracting officer will be considered inclusive.  The contractor shall provide unlimited repair service visits on the Government owned units. The contractor shall respond to emergency requests for service by telephone within two hours of notification. This emergency request will come from the end user, COR or other Biomed personnel. If the issue cannot be corrected over the phone with the FSE providing technical support, the FSE shall commence on-site repairs within the next day of the emergency service request. The contractor shall immediately, but not later than 24 consecutive hours after discovery, notify the COR in writing of the existence or the development if any defects in, or repairs required to the scheduled equipment that the contractor is not responsible for under the original terms. The COR shall review the additional services prior to the submission of any quotes or before additional work begins. D. OTHER REQUIREMENTS: The contractor shall: Be a firm regularly engaged in the repair, maintenance, and servicing of Alcon equipment described in this Statement of Work. Have expertise, experience, and regular business to include the repair, maintenance of the equipment. Possess all the manpower, equipment, tools, and ready sources of parts as necessary to carry out the conditions of the requirement promptly, skillfully, and expertly Contractor shall complete VA s Information Security and Privacy Awareness training course annually SERVICE REPORTS: The contractor shall be required to report to Biomedical Engineering and/or Surgical Service for a mandatory log in. Upon completion of service, the contractor shall document services rendered on a legible Vendor Engineering Service (ESR). The vendor shall log out upon completion and submit the ESR(s) to Biomedical Engineering and/or Surgical Service. All ESRs shall be submitted to the equipment user for an acceptance signature and to the COR for and authorization signature. If the COR is unavailable, a signed authorized copy of the EST shall be sent to the COR by the contractor after the work can be reviewed. Documentation shall include the following: Name of Contractor Name of FSE that performed services Contractor Service ESR/Log number Beginning/ending dates and times Equipment Downtime and Hours On-Site for service call. VA PO#(s) covering the call, if outside normal working hours. Description of Problem Reported by COR/User. Identification of Equipment to be serviced: INV. ID#, Manufacturer s Name, Device Name, Model #, Serial #, and any other Manufacturers identification numbers. Itemized Description of Service Performed (including Cost associated with after normal working hour services), including: Labor and Travel, Parts (with parts #) and Materials and Circuit Location of problem/corrective action. Signatures: FSE performing services described. Employee who witnessed service described. Equipment downtime SAFETY REQUIRMENTS: The contractor performing the work must be certified by the OEM to perform preventive maintenance, repairs, and all required safety and functional verification checks on Steris equipment according to OEM standards. A copy of this contractor s valid and current certification must be provided to the COR or their designee upon request. The contractor must ensure that the equipment functions in conformance with the latest editions of NFPA-99, OSHA, CDRH, and OEM standards. If the contractor acts out of compliance with the foregoing requirements, the Contracting Officer or their designee will notify the contractor of any noncompliance and action to be taken. The contractor shall, upon receipt of such notice, immediately correct the conditions which led to the act of noncompliance. Such notice, when served on the contractor or their representative at the work site, shall be deemed sufficient for the purpose aforesaid. If the contractor fails or refuses to comply promptly, the contracting officer (CO) may issue an order stopping all and any part of the work and hold the contractor in default. INSPECTION: The COR shall perform quality assurance inspections to verify the performed work. These inspections may not occur every service date. If the COR completes an inspection on a service date it will be in the presence of the contractor prior to the contractor s site departure. The contractor shall inform the end user and COR when any maintenance is completed. The COR may appoint a designee to be notified in the event that the COR is not present during the service day. The contractor shall accept responsibility for the equipment in as is condition. Failure to inspect the equipment prior to contract award will not relieve the contractor from performance of the requirements of this contract. FEDERAL HOLIDAYS: Federal Holidays which are observed by the Federal Government are annotated below: - New Year s Day - Martin Luther King Jr. Day - President s Day - Memorial Day - Juneteenth - Independence Day - Labor Day - Columbus Day - Veteran s Day - Thanksgiving Day - Christmas Day And any other day specifically declared by the President of the United States to be a national holiday. PERIOD OF PERFORMANCE: The base period of performance will be one (1) year from the date of award with provision of four (4) Option Years. All service shall be performed during normal business hours of coverage unless proper alternate arrangements have been coordinated beforehand with the COR. Work hours will be from 8:00 AM - 5:00 PM, Monday-Friday, excluding Federal Holidays.
