Description
STATEMENT OF WORK (SOW) Contract Number: Task Order Number: IFCAP Tracking Number: Follow-on to Contract and Task Order Number: 1. Contracting Officer s Technical Representative (COTR). Name: Donald Sedlacek Section: FMS / Biomedical Address: 500 W Fort ST, Boise ID 83702 Phone Number: 208-422-1594 E-Mail Address: Donald.sedlacek@va.gov 2. Contract Title. FAST 7500 Qualification and Maintenance 3. Background. The 7500 FAST DX with SNs 711-81245 and 711-85534 are used by the Boise VAMC lab for Covid testing. The system is no longer under warranty, so a new service and maintenance contract is required to ensure that technical support, qualification, and device repair and replacement are available when needed. 4. Scope. This is a service contract covering both 7500 FAST system devices with SNs 711-81245 and 711-85534 that used in the Lab at VA Boise. The period of performance starts from 12/23/2021 to 12/22/2022 and includes four option years. The systems are to operate in accordance to manufacturer specifications and tolerances. All parts replaced are to be OEM or OEM approved parts and components. Contract to cover all preventive and corrective maintenance as outlined in tasks below. 5. Specific Tasks. Contract shall provide: -Remote support during business hours, response within 1 hour -On-site maintenance Monday-Friday 6am-10pm MST, response within 2 business days -Annual preventative maintenance in accordance with maintenance schedule -Replace parts as necessary at contractor expense Contractor shall bill on one monthly invoice; each covered System, or Device, shall be a separate line item of the monthly invoice. 6. Performance Monitoring: Performance will be monitored by periodic inspections and observation of work. Work will be observed by the CO s POC, or designee, during performance of scheduled maintenance service and inspections and/or during repair service. Equipment operators will be contacted to report on contractor performance when CO s POC, or designee, is not available. 7. Security Requirements: The contractor shall have no access to VA sensitive information or to a VA internal trusted network. The C&A requirements do not apply, a Security Accreditation Package is not required. VA personnel shall act as an escort whenever the contractor is on site. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). No Government supplied Equipment or Information is required or anticipated. 9. Other Pertinent Information or Special Considerations. Only Manufacturer Trained Service Engineers/Technicians or Manufacturer Authorized Service Engineers/Technicians will perform services and repairs, when required. a. Identification of Possible Follow-on Work. No Follow-on work is anticipated. b. Identification of Potential Conflicts of Interest (COI). No Known COI c. Identification of Non-Disclosure Requirements. Contractor will not have access to Patient Records, Network Access and Sensitive areas. d. Packaging, Packing and Shipping Instructions. N/A e. Inspection and Acceptance Criteria. CO s POC, or designee, will verify that the unit is working within Manufacture s specifications after completion of service or repair. 10. Risk Control. The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractor s fault or negligence. The Contractor will not discuss any patient issues that were observed during the course of the work. The Contract will comply with VA requirements for VA Sensitive Information and VA Privacy Issues while performing any work on site. 11. Place of Performance. All on site service will be performed at the Boise VAMC. 12. Period of Performance. 12/23/2021 to 12/22/2022 plus four option years 13. Delivery Schedule. Service Reports will be emailed or faxed to CO s POC, or designee by the 5th workday after completion of the work. Billing Invoices will be sent monthly in arrears to the Austin Financial Operations Center.
