Description

SOLICITATION INSTRUCTIONS W81K00-21-Q-0173 Solicitation Amendment is being issued due to the Government's requirement changing to Zenith Verti-Lift Model 460 Thompson Elevation/Hylo with Auto Cocking AIR Drops Chiropractic Table. Attached is a solicitation amendment for Zenith Verti-Lift Model 460 Thompson Elevation/Hylo with Auto Cocking AIR Drops Chiropractic Table, Delivery, and Installation for General Leonard Wood Army Community Hospital, Fort Leonard Wood, Missouri, 65473. In order to be considered for the proposed contract the appropriate contractor must: 1. Fill out pricing as appropriate for each of the items listed in the CLIN structure on the Attachment 1 Price Schedule and Salient Characteristics: CLIN 0001 provide pricing for each unit and the total amount. CLIN 0002 provide pricing if this is applicable. CLIN 0003 provide pricing for each unit and the total amount. CLIN 0004 provide pricing for each unit and the total amount. � Please note there are special instructions in the revised 52.212-1 ADDENDUM. These must be adhered to or the vendor will not be considered for award. � Please read the revised 52.212-1 ADDENDUM and the revised 52.212-2 ADDENDUM carefully. 2. Proposed vendor MUST SIGN and DATE in blocks 15 a, b, and c. Please return the completed solicitation amendment, Attachment 1 Price Schedule and Salient Characteristics, and Quote�via email to fredicinda.d.jones.civ@mail.mil NLT 08 September 2021 by 10:00 a.m., Central Standard Time as noted in Block�14 of the solicitation amendment.
