Description

This is a SOLICITATION NOTICE for Solicitation Number 75H701-21-R-00025 for the Phoenix Indian Medical Center (PIMC) Elevator Modernization (Phoenix, AZ), a construction project for the Indian Health Service, Division of Engineering Services.� The full scope of work, plans and specifications and Davis Bacon Wage Determination are included in the solicitation documents.� Amendment A00001 posted 5/19/2021. Amendment A00002 posted 5/24/2021. Amendment A00003 posted 5/26/2021. Amendment A00004 posetd 6/4/2021. NOTE:� On May 24, 2021, the https://sam.gov/ functionalities will merge with the https://beta.sam.gov/ environment, at which time the two merged systems will be known only as https://sam.gov/. SCOPE OF WORK:� The project will include the modernization of four (4) elevators in the main hospital building of PIMC.� The elevators are 1979 Dover passenger elevators that serve six (6) floors at PIMC.� The work includes the removal and disposal of elevator components not being used in the modernization, the supply and installation of new components, refurbishment of components, cab interior refurbishment, electrical work, preventative maintenance during construction and warranty periods, minimum one year warranty, and all other work necessary to provide fully functional elevators in compliance with ASME A17.1 - 2007 Safety Code for Elevators and Escalators.� The successful Contractor shall coordinate with PIMC�s access control vendor for access controls on designated elevators and PIMC�s fire alarm vendor for any required fire alarm initiating devices per NFPA 72 standards. LOCATION OF PROJECT:� All work shall be performed at the Phoenix Indian Medical Center, which is located at 4212 North 16th Street, Phoenix, Arizona 85016. CONSTRUCTION DURATION:� Period of Performance is 300 calendar days from issuance of the Notice to Proceed. CONSTRUCTION MAGNITUDE: The magnitude of this construction project is anticipated to be between $1,000,000 and $5,000,000, in accordance with FAR 36.204(g). SITE VISIT:� An organized site visit has been scheduled for Friday, May 14, 2021 @ 10:00 AM MST (unless amended).� Refer to the Solicitation, Section L, FAR 52.236-27. PRE-PROPOSAL INQUIRIES/QUESTIONS:� Due no later than 10 calendar days prior to the proposal due date. PROPOSAL DUE DATE: �June 17, 2021, 2:00 PM MST (unless amended) This project will be procured as a 100% Small Business set-aside solicitation under North American Industry Classification System (NAICS) code 238290 Other Building Equipment Contractors.� The associated size standard for this procurement is $16.5 million. �For the purposes of this procurement, a concern is considered a small business if its average annual revenue for the last three (3) years is less than $16.5 million. �For information concerning NAICS and SBA size standards, go to http://www.sba.gov. It is the responsibility of the contractor to check https://beta.sam.gov/ frequently for any amendments or changes to the solicitation.� Hard copy documents will NOT be available � all documents for proposal purposes will be posted at the website for download by interested parties. This solicitation will be procured as a Request for Proposals (RFP) in accordance with FAR Part 15 and 36 procedures with Lowest Price Technically Acceptable evaluation criteria. CONTRACT TYPE: �The Indian Health Service intends to award a Firm Fixed Price (FFP) construction contract in support of this requirement. NOTE: Registration in the System for Award Management (SAM) database is mandatory to be considered for award. �Offerors may obtain information on registration and annual confirmation requirements via the internet at: https://www.sam.gov. �Contractors are encouraged to complete SAM registration as soon as possible; the government may not delay award for the purpose of allowing a contractor additional time to register in SAM.
