Responses to this Sources Sought must be in writing. STATEMENT OF WORK (SOW) FOR Robert J Dole VAMC 1. INTRODUCTION This is to provide the Veterans Health Administration, Wichita Audiology Department with a full-service agreement, to provide maintenance and repairs for equipment within the department. 2. BACKGROUND The Wichita Audiology Department currently uses medical equipment to measure and diagnose patients. This equipment needs regular scheduled maintenance to ensure proper operation. The equipment is located on the 4th floor of the main hospital and a few items at our outside clinics located in Hutchinson, Parsons, Salina, Hays, and Dodge. 3. SCOPE OF WORK The contractor shall provide scheduled Maintenance and calibration services on site, to include all labor, travel expenses, tools, test equipment and other incidentals necessary to maintain or restore the listed equipment to operating specifications which meet or exceed the manufacturers specification. Equipment is to be checked on an annual basis except for the items with a model of Tympstar Pro which will be checked sermi-annually. No repair services will be rendered under this contract: if repair services are needed a separate purchase order action will be initiated. Healthcare Technology Management (HTM) will coordinate with contractor for scheduling calibrations or preventive maintenance. Calibrations Preventive maintenance inspections shall be performed in accordance with manufacturer s literature on a Annual basis during the month of May (CBOCs and Main Facility) and the Tympstar Pro models on a semi-annual basis during the month of May and November (regular calibration and PM) services must be prescheduled to avoid equipment access conflicts. Calibrations and preventive maintenance inspections shall be performed in a manner which clearly demonstrates that all equipment meet or exceed all manufacturers technical performance specifications for preventive maintenance. Each calibration and preventive maintenance inspection shall include all items outlined in the manufacturers manual. Inspections shall be performed between 8:00 a.m. and 5:00 p.m., Monday through Friday, excluding federal holidays. Preventive maintenance inspections shall be scheduled with Biomed staff, giving a minimum of three (3) normal working days advance notice prior to arrival. A preventive maintenance inspection may be combined with an emergency service call only when advance notice has been given. The Contractor must inform Biomed at the time of inspection that a combined service and inspection is to be performed. 4.
