Description
HOOD CERTIFICATION/MAINTENANCE STATEMENT OF WORK - RESEARCH SERVICE 589-21-3-8101-0282 The C&A Requirements do not apply and a Security Accreditation Package is not required. Contractor shall provide all parts, tools, test equipment, software, labor, travel and equipment necessary to provide inspections, perform preventive maintenance (including cleaning) and certifications of Chemical Fume Hoods, Class II Biological Safety Cabinets (Type A and Type B), Laminar Flow Units, Clean Rooms, and Barrier Isolators located in Research Service at the Kansas City VA Medical Center. This work shall be accomplished on a semi-annual basis. Contractor shall ensure that equipment services by this contract functions in compliance with the most up-to-date published edition of the applicable standards including the NSF 49, ANSI/SEFA, ASHRAE 110, USP 797, CETA CAG-002, NIH EPA, JCAHO, UL, OSHA, CDRH and VA standards. The contractor will be responsible to ensure that Contractor Employees providing work on this contract are fully trained, NSF accredited and completely competent to perform the required work. Equipment used in performance of the contract by Contractor Employees will function in conformance with the latest published edition of the applicable standards including the NSF 49, ANSI/SEFA, ASHRAE 110, USP 797, CETA CAG-002, NIH EPA, JCAHO, UL, OSHA, CDRH and VA standards. All equipment tested shall be properly labeled/tagged after certification completion prior to leaving site. Semi-Annual on site certifications for Biological Safety Cabinets (BSCs) must be accomplished in accordance with the most current National Sanitation Foundation/ American National Standards Institute (NSF/ANSI) Standard 49 Class II (laminar flow) Biosafety Cabinetry. The list of tests includes, but is not limited to: Downflow Velocity and Volume Test HEPA filter leak test Cabinet leak test Inflow velocity test to include exhaust airflow volume rate Airflow Smoke pattern test Electrical leakage and ground circuit resistance and polarity tests Lighting intensity test Vibration test Noise level test Ultraviolet (UV) lamp test Laminar Flow Unit Certifications must be accomplished in accordance with the most current version of the National Sanitation Foundation/American National Standards Instituted (NSF/ANSI) Standard 49 class II Bio-Safety Cabinet. Fume hoods certifications must be accomplished in accordance with the most current version of the American Conference of Government Industrial Hygienists (ACGIH), Industrial Ventilation: A Manual of Recommended Practice. Halogen Leak Test must be performed in accordance with VHA Handbook 1106.1 Pathology and Laboratory Medicine Service Procedures . A copy of the handbook may be found at https://www1.va.gov/vhapublications/ViewPublication.asp?pub_ID=1779 Contractor Check-in The contractor s representative will schedule all work for Research Service in conjunction with the work done for the Kansas City VA Hospital Facilities. Contractor s representative will contact the Budget Analyst in Research at 816-861-4700 ext 57062 or 52755 prior to performance of the services. Contractor will report all issues to the Contracting Officer s Technical Representative (COTR) for Research Service while performing work for Research Service Kansas City VA Medical Center. The contractor must furnish a detailed field service report or equivalent showing work completed, listing all repairs and/or repair parts used to maintain operating efficiency of the equipment. This report must be delivered to the COTR (or designee) as the contractor signs out and leaves the facility or sent via email within 3 business days. Payment will be withheld pending receipt of this report. The service report from the contractor must contain, at a minimum, the following information: Date and time of the contractor s arrival on station Site or serial number(s) of equipment on which maintenance was performed. Total time spent performing maintenance, including travel time. Detailed narrative description of the malfunction or PM action required. Complete list of parts replaced. Comments as to the cause of the malfunction when applicable Date and time the repair or Pm was completed. The contractor s filed service employee shall wear visible identification at all times while on the premises of the VA Medical Center. Smoking is prohibited inside of buildings and only allowed in designated areas. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search, Violation of the VA regulation may result in citation answerable in the United States (Federal) District Court. All vehicles will be locked and the keys removed while performing service on the Medical Center s property. This is intended to protect the contractor s property and provide for safety of the Medical Center s personnel. Vehicle(s) shall be identified. Parking in the appropriate designated parking areas is expected. Information on parking is available from the on-site VA Police. The VA Medical Center will not invalidate or make reimbursement for parking violations of the contractor for any condition. In performance of this contract, the contractor shall take such safety precautions as the Contracting Officer or designee may determine to be reasonable necessary to protect the lives and health of the occupants of any building. The Contracting Officer or designee will notify the contractor of any non-compliance with the foregoing provisions and action to be taken. There is a potential for exposure to blood-borne or other infectious material with equipment throughout the hospital. All maintenance persons must use the Universal Precautions (i.e. decontamination of medical equipment, wearing protective gloves, aprons, and goggles) during the cleaning and maintenance/repair procedures required by this contract. BSCs that have been used for work involving infectious materials must be decontaminated before HEPA filters are changed or internal repair work is done. The contractor shall post signs at the door to the laboratory indicating that decontamination is in progress and entry is limited. In addition, a warning sign should also be displayed at the face of the BSC during the decontamination procedure. The contractor does not require services that involve connection of one or more contractor-owned IT devices (such as a laptop computer or remote connection from a contractor system) to a VA internal trusted (i.e., non-public) network.
