Description
The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, TX, intends to issue solicitation�15B51821Q00000010 for the provision of Optometrist Services.� The services will be performed at the Federal Correctional Complex (FCC), Health Services Department, 1507 East Whatley Road, Oakdale, Louisiana 71463. The FCC includes the Federal Correctional Institution I (FCI I). Federal Correctional Institution II (FCI II), and Federal Prison Camp (FPC). The employees will sign the consultant log, indicating the date of visits to the institution, the time of arrival and departure. �The Contractor shall adhere to all standards and regulations associated with the Accreditation Association for Ambulatory Health Care (AAAHC), American Correctional Association (ACA) Core Standards for Adult Correctional Institutions, all Bureau Policy, Clinical Practice Guidelines and Local Policy and Procedures.� Only one Optometrist is needed to fulfill this requirement.� There will be an estimated 48 sessions per month or 576 sessions per year.� The sessions will occur Monday through Friday between the hours of 7:30 a.m. and 4:00 p.m., excluding Federal Holidays. Each daily schedule will include a 30-minute unpaid lunch break. One (1) session is defined as one (1) hour in length. These hours may be adjusted to meet the variable needs of FCC Oakdale and the unpredictable nature of the correctional environment. The contractor shall normally not exceed 48 hours per month. The number of visits to the institution to perform the required services will be an estimated 6-10 times monthly. The contractor should normally evaluate an estimated 20 to 35 patients per visit depending on the length of the visit etc. Specific dates and times of sessions will be determined mutually between the contractor and the Government. The anticipated date for receipt of quotes is on or about June 18, 2021. Faith-Based and Community-Based organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. Insert the clause FAR 52.204-13, System for Award Management Maintenance, in solicitations that contain the provision at 52.204-7, and resulting contracts. This clause requires all contractors doing business with the federal government after September 30, 2003, to be registered in the SAM database at https://beta.sam.gov/.� Contractors should include their Data Universal Number System (DUNS) number in their offers. The Contracting Officer will verify registration in the System for Award Management (SAM) database prior to award by entering the potential awardees DUNS number into the SAM database. The acquisition is total small business set-aside. The North American Industrial Classification (NAICS) code is 621320 with a small business size standard of $8 million.
