- *THIS IS A TOTAL SMALL BUSINESS SET-ASIDE The Indian Health Service (IHS) is requesting quotes in accordance with the requirements of �Request for Quotation 75H70921Q00001 for Annual Preventive Maintenance Services on the Optometry�Equipment at the IHS facilities in the Billings Area (Montana and Wyoming). The procurement was�conducted pursuant to the authority of Federal Acquisition Regulation (FAR) Subpart 12.6 -�Streamlined Procedures for Evaluation and Solicitation for Commercial Items, FAR Part 13.5,�Simplified Procedures for Certain Commercial Items; and FAR 37.101, Non-personal Services Contract.�The purchase order is for 12 months with four 12-month options. Each PM Service is inclusive of�labor, materials, supplies, equipment, travel, and all applicable federal, state and local sales�taxes, et cetera. See attached equipment list of existing equipment and the number of exam rooms�noted below, this is subject to change with standard clinic upgrades and planned remodels. �An�updated equipment list and exam room list will be provided annually if changes occur. The NAICS code is 811219 and the size standard is $22M. PERIOD OF PERFORMANCE Performance of this purchase order shall be for 12 months est. June 1, 2021, through May 31, 2021, with four 12-month options. If the Government exercises the options pursuant to Federal Acquisition Regulation (FAR) 52.217-9, Option to Extend the Term of the Contract (MAR 2000), the period of performance shall be extended in accordance with the following schedule: STATEMENT OF WORK 1. The Contractor shall provide annual preventive maintenance on the optometry equipment in the Optometry examination rooms at the IHS facilities in the Billings Area (Montana and Wyoming). 2. The trained technician shall clean, adjust, lubricate, and calibrate all of the exam room equipment 3. Provide exam room lane software updates and IT troubleshooting as needed. 4. The trained technician shall clean, adjust, update software, calibrate and repair all requested ancillary testing equipment. 5. Provide on-call services and IT troubleshooting for equipment malfunctions. PM Services on Phoroptor: 1. Cleaning service on all phoroptors: The Contractor shall provide a loaner phoropter while cleaning each phoropter. The phoroptors may be cleaned on-site or taken/shipped off-site. 2. Clean and lubricate all mechanical parts, reposition correctly and adjust smooth phoroptors. 3. Thoroughly clean and check all optics (lenses and prisms). 4. Clean and align all scales. 5. Lubricate and adjust main support PD and convergence controls, reading rod arm and top locking device. 6. Clean, lubricate and align loupe turrets, including Jackson cross cylinders and prism assemblies. 7. Adjust all control knob tensions to correct smooth operating industry standards. 8. Contractor will provide each service location with an annual preventative maintenance service report. 9. Phoropters and equipment will be tagged annually with a maintenance sticker. 10. Additional repair services, as needed, to cover any malfunctions that occur outside factory warranty and preventive maintenance. Quotes must contain the following documents in order to be considered responsive and eligible for Award; Completed and Signed 1449 Capabilities Statement that technicians are fully trained and competent service the optometry equipment Capability to meet delivery or performance schedules as listed in the Scope of Work Completed Reps and Certifications (52.212-3 Offeror Representations and Certifications-Commercial Items. (FEB 2021)) Completed Past Performance Questionnaire Liability Insurance Provided upon Award 52.212-2 Evaluation-Commercial Items. (Oct 2014) ����� (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award shall be made to the Lowest Price, Technically Acceptable Offer. The following factors shall be used to evaluate offers: Capability to meet delivery or performance schedule as outlined in the Scope of Work Successful Past Performance in providing services to listed equipment Statement of Capability of Technicians to provide competent service to various types of optometry equipment Ability of Contractor Personnel to complete a background check; ����� (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). ����� (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer�s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
