- This is a sources sought notice 36C24421Q0697; this is neither a solicitation nor a promise of future work. The Pittsburgh VAMC is seeking SDVOSB/VOSB and any small or large company regardless of Socio-economic category that can provide the requirement as spelled out in the Statement of Work (SOW) and list of items below. The veteran business must be registered in betaSAM and certified in VETBIZ (VIP). The small or large businesses must be registered in betaSAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have permission to provide and install any systems or software. Are you a manufacturer, dealer, or distributor? New medical equipment will be required, all resellers, dealer, distributors must be an authorized distributor and provide an authorized distributor letter from the Original Equipment Manufacturer (OEM) stating that fact. Responses shall be submitted electronically, via email to Donna Toliver. Email address is donna.toliver3@va.gov. Responses are due no later than Thursday 5/20/2021 at 10:00 am. STATEMENT OF WORK: Symphonie Aqua Compact/Travel System 1.0 GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for the Cypress Adaptive Symphonie Aqua Compact/Travel Casting System. This system of casting makes it possible to recreate the loading forces on the residual limb exactly as they would be inside a prosthetic socket under actual full weight bearing conditions, producing an accurate and properly fitting plaster impression. This system is a hydrostatic pressure casting device that allows an impression of the actual anatomy of the connective tissue and bony structures of the residual limb. Patients can apply their entire body weight, allowing for the capture of the residual limb underweight bearing conditions. This method of hydrostatic pressure casting facilitates maximal clinical efficiency by reducing or eliminating the need for cast modifications and multiple check socket fittings. Equipment will be utilized in the Orthotic and Prosthetic Clinic- Heinz building 71 as well as used on the Prosthetics owned mobile repair van. 2.0 PERFORMANCE REQUIREMENTS The contractor shall provide and deliver Symphonie Aqua Compact/Travel Casting system to the Pittsburgh VA, Building 49, Prosthetics, 1010 Delafield Road, Pittsburgh, PA 15215. Duty hours for delivery will be 7:30 am 3:00 pm, Monday through Friday. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. 5.0 PRODUCT General This is a brand name requirement. The brand name is preferred and is used as the basis of need. Equivalent products will be considered acceptable if they meet the outlined specifications below. System must be a hydrostatic pressure casting system System must be portable System must be able to complete casting while patient is weight bearing 6.0 SERVICES 1.0 General Scope: This requirement includes delivery of specialized Prosthetic equipment. The contractor shall provide equipment, delivery and set up support, if necessary, to satisfy the needs of the department. 3.0 General Requirements for Services: The dates and times for delivery and receipt of equipment will be established by award of a delivery order. Delivery and receiving will not occur outside of normal duty hours, referenced in Section 2.0, and will not interfere with patient care. Delivery Heinz Building 49, Receiving, is located on the campus and has a loading dock. The loading dock is approximately 9 wide x 18 long. Final acceptance Prosthetics Dept representative will unpack and inspect delivery upon arrival and sign off on receiving report.
