VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17 Revision 02 Eff Date: 03/31/2020 DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K) Acquisition Plan Action ID: 36C250-21-AP-3134 Contracting Activity: Department of Veterans Affairs, VISN 10, Dayton VA Medical Center 552-21-3-6123-1129 Maintenance and Operations Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The vendor will complete the following in the amount of $37,472.00 to make the necessary repairs to the brick façade: Construct a 38' H scaffold tower connecting to a 12' H X 45' L X 3.5' W scaffold on the upper roof over the atrium and a 4' W X 49' L X 21.5' H scaffold on the lower roof over the atrium. Provide Labor and Materials required for: Grinding and tuck-pointing inside walls, est. 80 LF Grinding and tuck-pointing brick where required, est. 60 LF Utilize O. C. mortar Remove existing caulk, est. 200 LF Re-caulk exposed joints with elastomeric materials to allow for expansion and contraction, est. 200 LF Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: It was recently discovered the façade at the entrance of building 330, Dental Clinic, has damaged mortar between the bricks. This is allowing water behind the bricks and causing damage to the building and electrical components for the automatic entry doors. The repairs are required immediately to prevent further damage which could significantly increase costs. Description of market research conducted and results or statement why it was not conducted: A VIP search was conducted and resulted in 11 SDVOSB vendors within the Ohio area. This vendor was one of the SDVOSBs indicated in the search. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
