Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written solicitation will not be issued.  Solicitation number 36C25721Q0823 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. SET-ASIDE:  This requirement is a Service Disable Veteran Owned Small Business and Veteran Owned Small Business Set-Aside, the NAICS is 339113 and the small business size standard is 1250 Employees. In order to be eligible an offeror must be small for the applicable NAICS code at the time of the quote submission and at the time of award. Only qualified offerors may submit bids. Introduction:  A Firm Fixed Price award will be made to the lowest priced quote which conforms to the requirements within this solicitation and represents the best price to the Government. Description of Services:  The Contractor shall provide all personnel, supervision, transportation, equipment as outlined in the Statement of Work. See the Statement of Work for full details. Place of Performance: Department of Veterans Affairs Medical, Amarillo Health Care System, 6010 Amarillo Blvd, Amarillo TX 79106 Type of Contract: A Firm Fixed Price Base Period of Performance: Base:                           TBD Site Visit: NA Instructions to Offerors Evaluation FACTORS for Award Evaluation Factors for Award I.          Basis of Award The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation meets or exceeds the requirement at the lowest price. The quotes will be evaluated on the basis of the evaluation factors listed below.  The area will be evaluated on an ""acceptable"" or ""unacceptable"" basis.  DEFINITION RATING Passes (or meets) minimum standard requirements Acceptable Fails to meet minimum standard requirements Unacceptable STATEMENT OF WORK THOMAS E. CREEK MEDICAL CENTER AMARILLO VETERANS HEALTH CARE SYSTEM (AVAHCS) 6010 AMARILLO BLVD. W AMARILLO, TX 79106 REPLACE FOUR (4) STAINLESS STEEL KITCHEN HOODS INCLUDING EXTINGUISHING SYSTEMS 1 PURPOSE To ensure the four (4) kitchen hood extinguishing systems operate according to NFPA 96 and Joint Commission EP 13 Guidelines. 2 WORK DESCRIPTION 2.1 The work consists of removing and replacing four (4) stainless steel kitchen hoods and their complete fire extinguishing systems. Testing and ensuring that the four (4) kitchen hood extinguishing systems operate according to NFPA 96 and Joint Commission EP 13 Guidelines. 3 GENERAL INSTRUCTIONS The Contractor shall provide copies of Certifications and State of Texas License per NFPA 96. The Contractor shall be responsible for all supplies, materials, equipment, labor, expert supervision, management and transportation to satisfactorily perform demolition and removal of and complete replacement for the existing four (4) kitchen hoods with their fire extinguishing systems. Contractors and Subcontractors shall follow CDC and State of Texas Guidelines regarding COVID-19. All personnel must be Screened and have Temperature checked before beginning each shift. Superintendent will be responsible for checking all personnel before work starts each day. Contractor will perform inspection and testing per NFPA 96 and Joint Commission EP 13 Guidelines.as listed below: 3.1 Remove and replace four (4) kitchen hoods to function correctly Per Local Fire Codes. 3.2 Quoted sequence of operation as follows: 1- Event of fire triggers Ansul System to discharge, 2- Exhaust fans are turned on, 3- Supply and make up air fans are turned off, 4- All powered appliances under hood are powered off and 5- Main gas solenoid is powered closed. 4 INCLUSIONS: 4.1 Work to be performed after 07:00PM to 05:00AM Monday thru Friday. 4.2 Demo and replace two kitchen hoods at a time to allow food service to continue on the other kitchen hood sides. 4.3 Provide a barrier on all sides to include a ceiling to keep dust and sanitation out of the operating side at all times. A closed air exhaust fan system will need to be used. 4.4 Start with the side that has Combination ovens first and then the side with the convection ovens. 4.5 Preferred time frame is 7 to 10 days per side. Contractor to supply approximate time to complete each side before work may begin. 5 OTHER REQUIREMENTS: 5.1 The Contractor will perform a survey of the existing facilities and perform an actual assessment of the existing areas. 5.2 All tools, materials and equipment will be strictly controlled by the contractor to prevent access to unauthorized personnel. All equipment must be secure when not in use. 5.3 Scheduling: The Contractor shall provide the work schedule for the service to be performed. The schedule will be part of the proposal package. One section of the schedule should include a breakdown of personnel required to perform the service. 5.4 Interference with Hospital Functions: Contractor personnel are required to Interrupt their work at any time so as not to interfere with hospital functions. This includes but not limited to, utility service, fire protection systems, and the passage of facility patients, personnel, equipment and carts. 5.5 Contractor Reporting: The Contractor Field Supervisor (CFS) will provide to the Contracting Officer (CO) an official company letter with a list of all personnel that will work on the project. The letter should provide the first name, last name and social security number. The COTR will make arrangement with the hospital police security section for the issuance of VA hospital identification cards. All contractors employees must wear the identification at all time while performing his/her duties on the installation. The identification must be visible in the torso area. Upon completion of the project all identification must be returned to the COTR who will return them to the Police Service section. Each card not returned or lost will generate a payment to the Government on the amount of $25.00. During the hours of 7:00 PM to 5:00 AM the CFS will report to the Security Police Supervisor on shift the number of personnel, their activities and the areas of operations. The COTR will coordinate parking and off load of supply and equipment arrangements with the police section and the Contractor. 5.6 Damages: Any wall surface, glass, floor tile, window or other material broken or damaged by contractor personnel during the performance of this project shall be corrected/repaired to the previous existing condition or better. 5.7 Disposal of Waste: All waste and rubbish materials are to be disposed off Medical Facility premises by the Contractor, and disposal shall be in accordance with Federal and local regulatory guidance and laws. 5.8 Safety: While in the performance of the work, the Contractors personnel shall abide by the safety requirements of the Medical Center. The Contractor must exercise extreme care in making the work area safe at all times. The number one priority is to protect patients and hospital staff from any possible injury resulting from the equipment or materials. The Contractor is responsible for security of equipment each day. There is no equipment storage area available on site. The hallways nor corridors shall not be used to store, pile or place construction materials or supplies. In the event of a situation requiring use of the corridor for such purposes, approval of the VA Medical Center Safety Manager/Officer shall be obtained previously. No material or supplies shall be placed where patients may recover them. Any safety violation or emergency situation should be reported to a hospital official immediately. The Contractor shall conform to all Federal, State and local regulations governing painting chemicals and equipment, which may be in effect. All equipment used to perform the work will conform to OSHA regulations. 6 ADDITIONAL REQUIREMENTS: 6.1 A detailed electronic inspection and test report complying with applicable NFPA 96 standards shall be provided upon completion. 6.2 Contractor shall provide a detailed electrical schematic of the Shutdown System. The following clauses and provisions apply to this solicitation: 52.209-5, Certification Regarding Responsibility Matters 52.212-3, Offeror Representations and Certifications-Commercial Items 52.212-4, Contract Terms and Conditions-Commercial Items 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items 52.217-8, Option to Extend Services The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days. Evaluations of Options (July 1990) Except when it is determined in accordance with FAR 17.206(b) not to be in the Government s best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation.  Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price.  Evaluation of options will not obligate the Government to exercise the option(s). (End of Clause) 52.217-9, Option to Extend the Term of the Contract a) The Government may extend the term of this contract by written notice to the Contractor within 60 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 (five) years. (b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement. (c) All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the Contractor's business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. (d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirement. (End of clause) 852.219-76 SUBCONTRACTING PLANS MONITORING AND COMPLIANCE. AS PRESCRIBED IN 819.7203(C) INSERT THE FOLLOWING CLAUSE: SUBCONTRACTING PLANS MONITORING AND COMPLIANCE (JUL 2018) (a) This contract includes FAR 52.219-9, Small Business Subcontracting Plan, and VAAR 852.219-9, VA Small Business Subcontracting Plan Minimum Requirement. (b) Accordingly, any contract resulting from this solicitation will include these clauses, unless the contract is awarded to a small business concern. The Contractor is advised in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) to assist in assessing the Contractor's compliance with the plan, including reviewing the Contractor's accomplishments in achieving the subcontracting goals in the plan. To that end, the support contractor(s) may require access to the Contractor's business records or other proprietary data to review such business records regarding the Contractor's compliance with this requirement. (c) All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the Contractor's business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. (d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor compliance with the subcontracting plan.
