- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document is attached. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-05. This solicitation is set-aside for Small Business concerns. The associated North American Industrial Classification System (NAICS) code for this procurement is 621910, with a small business size standard of $16.5 Million. The PSC is V225. The VA Network Contracting Office 2 (130 W. Kingsbridge Rd., Bronx, NY 10468) is seeking to procure Ambulance Services supporting eligible beneficiaries supported by the VA New York Harbor Healthcare System. All interested, eligible firms shall provide quotations based on the attached written solicitation. Please read carefully and note the requirements for submission (i.e. 52.212-1 Instructions to Offerors). Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. Offerors must complete the representations regarding certain telecommunications and services or equipment in accordance with FAR 52.204-24. All offerors shall submit the following: SF1449: Complete Block 17a: Offeror Name, Address, POC information, Telephone No., DUNS Complete Block 26: Enter Total Amount of offer Complete Blocks 30a/b/c: Self explanatory Section B.1 (page 4): Enter Contractor information at 1.a. (Firm Name, Firm Address, POC Name and Email) Section B.3 (beginning on page 23): Enter pricing information (Unit price is the individual price; Amount is the Unit price multiplied by the quantity). Carefully review E.1 52.212-1 Instructions to Offerors Commercial Items. At sub-paragraph (b) Submission of offers: paragraph 4: Submit a technical plan and documentation as specified. paragraph 6: Enter pricing information at Section B.3 (beginning on page 23); using an excel spreadsheet to develop this pricing information is recommended to ensure accurate math calculations. paragraph 10: Submit at least three recent and relevant references. Complete the Past Performance Questionnaires and submit with the offer or have the reference submit directly to the Contracting Officer via email. paragraph 12: Complete the Limitation on Subcontracting Certification and submit with the offer. All offers shall be sent to the VA Network Contracting Office 2 Point of Contact via email at William.Mills4@va.gov. Hand or physical mail delivery of offers will not be accepted. Submission of your response shall be received not later than May 26, 2021 at 4:00 pm ET at William.Mills4@va.gov. Offerors are advised to be mindful of the size of the email submission(s) and may submit the offer in parts. Offerors are responsible for ensuring the Point of Contact received the email(s). Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Offerors shall list exception(s) and rationale for the exception(s), if any.
