PRODUCT CODE: ST010-L STAXI STANDARD CHAIR BLUE, Qty 50 EA PRODUCT CODE: STEX-COMP1M STAXI 1 YEAR FREE EXTENDED WARRANTY - (EXCLUDES CUSHION), Qty 50 EA PRODUCT CODE: ST059 CUSHION - ONE PIECE- BLACK- MEDICAL & COMMERCIAL, Qty 50 EA PRODUCT CODE: ST044 NEW OPERATIONAL SIGN (800X500), Qty 1 EA PRODUCT CODE: ST0159 STAXI TOOL KIT, Qty 1 EA PRODUCT CODE: ST4040 HANDLING, Qty 1 EA The manufacturer is STAXI CORPORATION, DUNS number 078405541. The applicable NAICS code for this procurement is 339112 with a business size standard of 1000 employees. Staxi Corporation is the only company that manufactures and distributes Staxi transport chairs. There currently are no agents or dealers outside of the company that are authorized to provide this product. This product is a medical transport chair; non-collapsible (no folding), steel frame, non-removable footrests and parts, 600lbs capacity for standard chair, designed to nest into each other saving valuable space yet ease in access, anti-tip mechanism, automatic brakes with handle control, storage space below seat area, composite wheels, and anti-tip mechanism. With full steel framing the ease of wipe down for sanitary requirement assists greatly in turning around the unit to the next patient. This notice of intent is not a request for competitive proposals. However, the Government will consider all proposals received within 2 days (NLT) February 12, 2021 at 4:00p.m. ET) after date of publication of this notice. A determination by the Government not to compete with the proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Attached is a copy of the approved justification and approval notice that supports the sole source award. System for Award Management (SAM): Rule requires a firm to be registered in the SAM Registration Data Base (SAM), prior to award of any contract and agreements.
