- 11 DEC 20 3RD UPDATE It was discovered that no dimensions for the ""Longside"" acrylic sheet - the measurement was only on the attachment with the drawing.� The purpose of this amendment is to add the dimensions to Part 5 of the PWS and to the PRS Technical Exhibit.� The due date is being extended to 16 DEC 2020.� [See updated combined synopsis/solicitation document with today's dated of 201211] End of 3rd Update Changes. 7 DEC 20 2ND UPDATE Forgot to add change to PWS Para 5.8 PRS#7 [Changed one of the desk panels to 33"" from 60""] Updated page 1 of the Past Performance Questionnaire Document for clarity. End of 2nd Update Changes 7 DEC 20 CHANGES START NOTE:� Do not forget to submit your past performance questionnaires with your quotes Installation Time Frame Change:� Changed from 20 Days to 10 Business Days [Original Document had both time frames] Hours of Operation:� Installation is to be during normal business hours b/w 8:00 AM ET to 5:00 PM ET Long Side Sheet:� Change the 7 inch cut to a 6 1/2 inch cut. Urban Search and Rescue Office Space:� Those two sheets referenced in PWS Para 5.13 and PRS # 12 are to be 3/8 inches thick Revised PWS is uploaded in Revised Combined Synopsis Solicitation Document NIMAT Floor Plan is uploaded with this change FEMA Basic Security Clearance NCIC Request Form uploaded [Complete and Submit with Quote] Questions and Answers Q:� What is the preference for securing the glass to the desks/fixtures?� A:� Secure the panels with a bracket/clamp that can be removed at a later date without damaging the furniture.� Q:� What material are the�stations made from to which the panels will be attached? A:� Most of the stations are wood, however there were a couple of metal items. Q:� Will the Government conduct the police background check with information we provide or do you want us to present the checks upon award? A:� FEMA will conduct the checks.� Provide the installers information on the attached/added spreadsheet and send it in with your quote. Q:� Are there any personnel number limits for installers? A:� No. [The end of changes and questions and answers for the 7 DEC 2020 Change] This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.� The solicitation number is 70FA4021Q0PLEXIGLASS01, and is issued as a Request for Quote (RFQ).� This solicitation document and incorporated provisions and clauses are those in affect through Federal Acquisition Circular FAC 2005-62.� The associated North American Industrial Classification System (NAICS) code for this procurement is 337215 with a small business size standard of 500 employees.� This requirement is a 100% Small Business Set-Aside, and only qualified offerors may submit quotes.� The items [82 40� high x 60� wide desk panels; 50 47� high x 69� wide long side panels; and 12 40� high x 30� wide short side panels of acrylic or plexiglass barriers to be installed at government location in Herndon VA] for this requirement will be listed in the below Performance Work Statement (PWS) document.� The acrylic/plexiglass sheets are to be installed within 10 business days after contract award.� There will be two site-visits at 11:00 AM ET and 1:00 PM ET on Friday, December 4, 2020 at 13992 Park Center, Herndon, VA, 20171 [FAR Clause 52.236-27 Site Visit (Construction)].� A mask will be required for entry, and there will be a quick temperature test prior to entry.� The provision at 52.212-1 [Instructions to Offerors-Commercial Items] applies to this acquisition.� The provision at 52.212-2 [Evaluation-Commercial Items] applies to this acquisition.� Any award resulting from this RFQ will be issued on Standard Form (SF) 1449 to the offeror whose quote is the lowest price technically acceptable with the highest past performance rating providing the Government the best value, and will contain all clauses required by law and the Federal Acquisition Regulation (FAR) as appropriate for the dollar value of the award.� Past Performance Questionnaires - bottom of attachment - from�Offeror�s previous customers are due at same time quotes from Offerors are due.� Offerors will provide a statement that the Offeror Representations and Certifications-Commercial Items [52.212-3 (52.204-8)] is up-to-date, and the offeror must registered in the System for Award Management database https://www.sam.gov/SAM/ when submitting an offer or quotation as required by 52.204-7(b)(1).� The clause at 52.212-4 [Contract Terms and Conditions-Commercial Items] applies to this acquisition.� The clause at 52.212-5 [Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items] applies to this acquisition.� Quotes are due no later than 11:00 AM Eastern ET 11 December 2020.� All quotes shall be submitted via e-mail to Davida Patton at davida.patton@fema.dhs.gov and Shawn M.
