Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. We are seeking information for market research purposes only. The Government will not pay for any information solicited. The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide the following item(s) for the Hunter Holmes McGuire VA Medical Center, 1201 Broad Rock Blvd., Richmond, VA 23249: Intellivue MMX B06 dual press, monitors and accessories by Phillips. All responses shall be submitted no later than 5pm EST Monday Dec 14, 2020. All interested parties should respond, in writing via email to Benetta.northern@va.gov. No telephone inquiries will be accepted. In response, please include the following: Company/individual name, Address, Point of contact with phone number Information describing your interest, Indicate FSS/GSA contract number or Open Market, as applicable FSS Category you qualify for Tax ID number DUNS number Capabilities statement Company Socio-Economic SIZE (example large, small, SDVOSB, VOSB, WOSB etc.) The NAICS 339112 is applicable to this acquisition; with a size standard of 750 Employees. This requirement may be subject to the non-manufacturer rule (please respond to each of the following statements): does not exceed 500 employees. ________ is primary engaged in the retail or wholesale trade and normally sell the type of item being supplied. ______ takes ownership or possession of the item with its personnel, equipment or facilities in a manner consistent with industry practice. _________ will supply the end item of a small manufacturer, processor or producer made in the US, or obtains a waiver of such requirement. ___________ Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award.
