- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is IHS1415646. The solicitation is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-09, effective October 26, 2020. The North American Industry Classification System (NAICS) Code for this acquisition is 611430. The Contract Line Items (CLINs) and pricing structure are as shown below: CLIN 1 COVID-19 Contact Tracing Training The Contractor shall provide all training materials, technical assistance and virtual training sessions using the CommCare Software System in accordance with the Scope of Work. Place of Performance:� Navajo Area Indian Health Service, 272 Hwy 264, Window Rock, AZ� 86515 Period of Performance: Date of Award through March 31, 2021 The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and the following addendum applies. Paragraph (b) entitled �Submission of Offers�: All responsible offerors shall submit the following documents NO LATER THAN 2:00 P.M. MST, November 18, 2020: Quote Technical proposal Past Performance Reference Fax submissions are not authorized. � 52.212-2, Evaluation-Commercial Items applies to this acquisition and the following addendum applies. The Government will award a firm-fixed price contract to the responsible offeror whose proposal represents best value. In determining best value, offerors will be evaluated in accordance to the following evaluation criteria: � Technical Capability � Offerors understanding of the services requirements: Minimum of 24 months of experience providing services of similar nature. Contractor shall show evidence of providing training utilizing the CommCare Software for contact tracing Past Performance � The Contractor shall provide evidence of providing services of similar nature within the past three years. Price Technical capability and price are of equal importance.� � Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.
