SINAI HOSPITAL LABORATORY.  Market research was conducted and determined that MT. SINAI HOSPITAL LABORATORY is the only vendor capable of providing specialized pathology for Fresh Tissue Neuromuscular Biopsies with Full Text Subscription necessary that meets the requirements.                  This Notice of Intent is not a request for Proposals or Quotations.  No solicitation document exists, and no telephone or email inquiries will be accepted.          No contract award will be made on the basis of proposals, or quotations received in response to this notice.          The government MAY consider all Expressions of Interest (EOI) received prior to November 11, 2020 by 10:00 AM EST.    A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government.  Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.    Responses to this notice must be emailed to Mariangie.Rivera@va.gov and received by 12:00P M EST on November 13, 2020.                   Submitters are responsible for ensuring their EOI reaches the designated Contracting Specialist on-time.                   See attached Statement of Work below for additional details.      If you have any questions or concerns, please contact me. STATEMENT OF WORK Neuromuscular Biopsy Fresh Tissue Services SCOPE The Northport VA Medical Center Pathology & Laboratory Medicine Service requires the performance of specialized pathology for Fresh Tissue Neuromuscular Biopsies which shall include paraffin sections from formalin fixed tissue, Cryomicrotomy sections from flash frozen tissue (liquid nitrogen), and Resin (EPON/plastic) Embedded sections for possible electron microscopy to be performed at the Icahn School of Medicine, Department of Pathology at Mount Sinai Medical Center, New York, NY. GENERAL REQUIREMENTS Muscle biopsies are a specialized procedure performed at a limited number of sites in the country. This is due to the need for a trained neuropathologist to review the specimens and the ability to perform electron microscopy. The tissues shall be sent by courier the same day of the procedure to the testing site in order to ensure that the tissue is examined without any loss of integrity. TESTING REQUIREMENTS In the below paragraphs, the following processes and stains on muscle tissue shall be submitted by the Northport VA Medical Center to the contractor. Not all processes and stains are required for each tissue submitted. The extent of service needed shall be determined by the contractor s pathologist and our Chief of Pathology. Paraffin sections from formalin fixed tissue: 88305 Hematoxylin & Eosin (H&E) Trichrome (special stain) 88313 Congo Red (special stain) 88313 Some cases: Possible immunohistochemistry for inflammatory markers (usually at least 5-6) 88342 and 88341x5 Cryomicrotomy sections from flash frozen tissue (liquid nitrogen): 88319 Hematoxylin & Eosin (H&E) Gomori trichrome (special stain) 88314 Periodic acid Schiff (PAS) (special stain) 88314 Periodic Acid Schiff with digestion (PASD) (special stain) 88314 Enzyme histochemical stains:88319 Nicotinamide Adenine Dinucleotide (NADH) 88319 Succinate Dehydrogenase/ Cytochrome oxidase (SDH/COX) 88319 Acid Phosphatase 88319 Alkaline Phosphatase 88319 Esterase 88319 ATPase 88319 Pre-incubation at 9.4 88314 Pre-incubation at 4.2 88314 Some cases: additional enzyme stains Resin (EPONTM/plastic) Embedded sections for possible electron microscopy: (EPONTM trademark epoxy resin) EPONTM (plastic section, stained with toluidine blue), typically x2 88313x2 Some cases: Electron Microscopy 88348 SPECIMEN PREPARATION AND TISSUE HANDLING The Northport VA Medical Center Histology Supervisor or designee shall notify the contractor the scheduled date of the procedure once the surgical case is scheduled. The surgical department or the surgeon shall advise that the specimen must be delivered to the Histology Department no later than 11:00 am to allow sufficient time for processing and transport of the specimen to the contractor. Testing shall be performed Monday through Thursday only. The contractor shall be notified by the Histology Supervisor or designee once the specimen is in route. COURIER SERVICES The Histology Supervisor or designee shall confirm transport with the designated courier service the day before the procedure and again the morning of the procedure to ensure timely pickup and delivery. Testing shall be performed at the contractor s local laboratory. RESULT REPORTING The contractor shall fax (secure fax) the final report within 10 working days for simple cases and within 20 working days when Electron Microscopy is required. The testing laboratory shall contact the Histology Department by telephone to 631-261-4400 ext. 2030. PERIOD OF PERFORMANCE The estimated period of performance is one base year with four option years.
