Peters VA Medical Center 130 West Kingsbridge Road Bronx NY 10468-3904 POC: Lateisha Robinson | Lateisha.robinson@va.gov DESCRIPTION: NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT TO Hologic for Ge-68 Phantom for Bronx VA Healthcare System. Market research was conducted and determined that Seimans Medical Solutions is the only vendor capable of providing supplies necessary that meet the requirements. POSITRON EMITTION TOMOGRAPHY (PET) Ge-68 Phantom PHARMACEUTICAL General Description:  Furnish all necessary resources to deliver one (1) new Ge-68 Phantom. Contractor shall provide a means to remove the expired isotopes after the unit has expired. PET/CT Germanium-68 Phantoms provide 2D and 3D normalization and test images of PET and PET/CT systems. The Ge-68 Cylindrical phantom will be used to verify the operation integrity of the Siemens Biograph PET/CT system at the Bronx VA. In addition, the phantom is needed to catch potential problems early, and maintain consistency, quantitative accuracy, and high image quality. The Ge-68 is important at the Bronx VA because a Siemens Biograph system is used to scan a high volume of patients. The Biograph system at the Bronx VA needs to have a quality check daily, which is performed by VA personnel. The results produced by the quality check determine the readiness of the system for scanning or whether service is needed. To run the daily quality check, the Bronx VA needs a Siemens Ge-68 phantom. The absence of Ge-68 will cause VA medical professionals to not be able to utilize the PET/CT scanner, which will cause a disruption in critically important patient care. The AOD will contact VA Police to accompany the delivery Person to the Nuclear Medicine Department and supervise the delivery. Deliveries will be supervised by the COR or their designee before beginning delivery operations. Contractor will contact the COR prior to delivery to arrange for delivery supervision. 3. Delivery Commitment: Time of delivery specified or mutually agreed to at the time of receipt of telephone order shall become mandatory upon the Contractor. Failure to perform in accordance with the delivery commitment may be grounds for termination of contract in accordance with the provisions for default. 4. Compliance in Removal Contractor shall certify in writing that it acknowledges and is in possession of all required business licenses and permits. The Contractor shall, without additional cost to the Government, provide and maintain all licenses and permits for operational personnel, trailers, containers, vehicles and other resources required for the hazardous waste disposal program in accordance with all applicable Federal, State, Municipal, and local regulations (e.g. OSHA, EPA, DEP, 49 CFR (DOT) including, but not limited to HM-18). Contractor shall provide a means to remove the expired isotopes after the unit has expired.
