Description
The Miami VA Medical Center is in need of contractor to provide autologous serum eye drops for patients. Each supply order will provide for 3 months of eye drops. The government estimates 150 orders per year. The desire is to award a contract that will provide eye drops in the most convenient manner possible and that phlebotomy services be sent to patients homes rather than patients traveling to clinic/blood bank. Basic work information is as follows: INTRODUCTION AND SCOPE OF WORK The Department of Veterans Affairs Medical Center, 1201 N.W. 16TH STREET, Miami, FL, 33415, (herein after known as VA), requires autologous serum eye drops (AST). The method of procurement shall be a firm fixed price contract for processing of AST for the period of 12 months. Contract length will be from one to five years. The contractor shall provide 3 month supply of AST customized for each patient. REQUIREMENTS Contractor must comply with the following: 1) Receive AST order from physician with patient demographics and specific order information. 2) Obtain blood samples from patient or from VA. 3) Process blood samples for AST under aseptic conditions, diluting as specified by physician s order. 4) Deliver 3-month supply of AST to patient. 5) Maintain quality program of AST to ensure patient safety. Qualified/interested vendors should email their company information to the Contracting Officer, which should include full company name, DUNS number, as well as verbiage that substantiates qualification to provide eye drops. Any questions should also be submitted in writing.
