- This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although ""proposal,"" ""offeror,"" contractor, and ""offeror"" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as ""proprietary"" or ""confidential,"" intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office in Buffalo, NY for the purpose of collecting information about Grease Trap Maintenance at the New Jersey Healthcare System at the East Orange Campus located at 385 Tremont Ave, East Orange, NJ 07018; and the Lyons Campus located at 151 Knollcroft Rd, Lyons, NJ 07939. The VA is seeking to provide for this requirement as of February 1, 2020. The NAICS code identified for this requirement is 562998 All other Miscellaneous Waste Management Services. The requirement is detailed in the Statement of Work section of this document. (e) Contractors that are able to meet the requirements in the below performance work statement are encouraged to email a capability statement and full information to Megan Ross at Megan.Ross3@va.gov. Email is the preferred contact method. - Part of the purpose of this sources sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. If contractor is a Service Disabled Veteran Owned Small Business (SDVOSB), a Veteran Owned Small Business(VOSB), or any other special category of contractors, then contractor is responsible for providing documentation in support of this. This information will be used to determine the viability of a set aside for this requirement. - Contractors shall also identify any Federal Supply Schedules that may carry the desired services. - Contractors shall identify pertinent point of contact for company, contractor DUNS number for size standard and socioeconomic verification in SAM and VIP. - Contractors shall include any relevant comments about the Attachment(s) if applicable. Statement of Work GENERAL INFORMATION: Grease Trap Cleaning and Maintenance at the following locations: New Jersey Healthcare System East Orange Campus 385 Tremont Ave East Orange, NJ 07018 & New Jersey Healthcare System Lyons Campus 151 Knollcroft Rd Lyons, NJ 07939 Period of Performance Base Year: 01/01/2021-12/31/2022 Option Year 1: 01/01/2022-12/31/2023 Option Year 2: 01/01/2023-12/31/2024 Option Year 3: 01/01/2024-12/01/2025 Option Year 4: 01/01/2025-12/01/2026 General Schedule: Contractor to provide 24 hour advanced notice to the Contracting Officer s Representative (COR) prior to commencing work. Normal working hours are 7am-3:30pm Monday- Friday except all federal holidays. Contractor shall notify staff upon arrival to be escorted to the work site. Lyons: Building 6 Grease trap & Bio-treatment station Building 54 Grease trap & Bio-treatment station Scope of Work: Notifications: Contractor shall provide a minimum of 24 hours advanced notice to the designated Contracting Officer s Representative (COR) prior to commencing work. Normal working hours are 7am-3:30pm Monday- Friday except federal holidays. Contractor shall notify designated point of contract upon arrival to be escorted to the work site. Contractor Required Tasks: Provide all labor, materials and equipment necessary to perform Bio-System treatment and removal to maintain grease traps in kitchen areas, vacuum and jet grease traps removing all liquids and grease from walls and tanks. Line Item 2 - Bldg. 54 Bio-System treatments for grease traps Monthly Invoices billed monthly. Line Item 3 - Bldg. 6 Interior pumping, transportation and disposal of waste from grease traps Quarterly in the months of March, June, September, and December. B Invoices billed in the months of April, July, October, and January. Line Item 4 - Bldg. 54 Exterior pump, transportation and disposal of waste from grease traps Quarterly in the month of March, June, September, and December. Invoices billed in the months of April, July, October, and January. Line Item 5 - Bldg. 54 Exterior jetting/cleaning of grease deposits will be performed Semi-annually in conjunction with Item 4 above in the months of June and December. Invoices billed in the months of July and January. Line Item 6 Bldg. 6 and Bldg. 54 Semi-Annually - Bio-Treatment batteries, provided by Contractor, to be changed twice a year in the months of June and December. Invoices billed in the months of July and January. F. CHANGES TO STATEMENT OF WORK Any changes to this Statement of Work shall be authorized and approved only through written correspondence from the Contracting Officer (CO). G. REPORTING REQUIREMENTS Service reports for the work performed are to be turned over to the COR. Reports must include the amount of substance removed from the pits for both Building 6 and 54. Service reports must be signed by both service provider and COR at completion of work. An electronic version of same shall be sent to the COR via email. H. CONTRACTOR EXPERIENCE REQUIREMENTS The Contractor employees shall possess OSHA 30 Hour Safety Training Card. The Contractor will be responsible to ensure Contractor employees providing the work on this contract are fully trained and completely competent to perform the required work. The Contractor shall provide the CO with copies of all certification documents. H. OTHER REQUIPMENTS Contractor must follow all state, city, and building policies.
