- Page 4 of 4 This is a Sources Sought Notice. (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although ""proposal,"" ""offeror,"" contractor, and ""offeror"" may be used in this Sources Sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as ""proprietary"" or ""confidential,"" intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office in Syracuse, NY for the purpose of collecting information about Metal Detector and X-Ray Machine Service and maintenance at the Bronx VA Medical Center (NY) The VA is seeking to provide for this requirement within 30 days of the award of the contract. The NAICS code identified for this requirement is 561621 Security Systems Services (except Locksmiths). The requirement is detailed in the Statement of Work section of this document. (e) Cursory market research has identified several small businesses that may be able to provide for this requirement. Contractors that are able to meet the requirements in the attachment are encouraged to email a capability statement and full information to Jason Rundle at jason.rundle@va.gov. Phone calls will not be accepted. - Part of the purpose of this Sources Sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. This information will be used to determine the viability of a set aside for this requirement. - Contractors shall also identify any Federal Supply Schedules that may carry the desired services. - Contractors shall identify pertinent point of contact for company, contractor DUNS number for size standard and socioeconomic verification in SAM and VIP. - Contractors shall include any relevant comments about the Attachment(s) if applicable. This Sources Sought Notice is for a service maintenance agreement for repair and maintenance of the metal detectors and x-ray security machines located at the Bronx VAMC Campus (address below) for a Base Period from 01/01/2021 through 12/31/2021 with the provision of four (4) option years. Facility: Bronx VAMC 130 West Kingsbridge Rd Bronx, NY 10468 STATEMENT OF WORK (SOW) X-RAY MACHINES AND METAL DETECTORS FULL SERVICE AND PREVENTATIVE MAINTENANCE SERVICE GENERAL: Contractor shall be responsible for allocating all equipment, labor, personnel, materials and supervision, to meet all requirements for the maintenance of the Security X-Ray Machines and Metal Detectors at the James J. Peters Veteran Affairs Medical Center. The Contractor must have relevant experience in providing maintenance services for metal detectors and x-ray systems. This contract will be specifically for the James J. Peters VA Medical Center located at: 130 West Kingsbridge Road, Bronx, NY 10468. BACKGROUND: James J. Peters VAMC is in possession of several security x-ray machines and metal detectors that are past their warranty periods. The facility is seeking a vendor that is qualified to do periodic preventative maintenance and emergency servicing for these devices. PERIOD OF PERFORMANCE: Base Year 01 January 2021 31 December 2021 Option Year #1 01 January 2022 31 December 2022 Option Year #2 01 January 2023 31 December 2023 Option Year #3 01 January 2024 31 December 2024 Option Year #4 01 January 2025 31 December 2025 MAINTENANCE SCOPE OF WORK: Contractor shall furnish all labor, parts, materials, supervision, equipment, supplies and tools necessary to fully maintain, service and repair the following list of equipment: Equipment List: QTY Model SN Location at Medical Center 1 Garrett PD6500i walk-through Metal/Weapon Detection Systems 58329366 Front Entrance 1 Garrett PD6500i walk-through Metal/Weapon Detection Systems 58142276 Front Entrance 1 Auto clear 5333 X-Ray System 5-8-29P-01 Mail Room (GB-18), Ground Floor Contractor shall perform periodic service and preventive maintenance/repairs as per the Original Equipment Manufacturer (OEM) recommended service schedule. This work shall be scheduled during normal business hours as outlined below in Section 5. Contractor shall support unlimited repair service calls to get the metal detectors and x-ray machines functioning properly, should they fail. Any service calls that require work beyond normal business hours, is considered after hours / emergency calls and are subject to overtime rates. Contractor shall be available to field calls 24/7 for emergency service/repairs/maintenance. Such calls shall be made by the Engineering Services Point of Contact (POC) or designee. The contractor shall commence work with a technician that is qualified to work on the equipment listed above within (4) hours after receipt of notification of emergency repairs/service. HOURS: Normal business hours, 8:00 am 4:30 pm Monday Friday (excluding Federal Holidays). The following are Federal Holidays observed by the facility and the Contractor shall not expect access to the VA Facility or VA employees on these days unless notified otherwise by the POC or Contracting Officer. Preventive maintenance or other services are not to be scheduled on these dates: New Year's Day Birthday of Martin Luther King, Jr. Washington's Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day SPECIAL CONTRACT REQUIREMENTS CONTRACTOR PERSONNEL The contractor shall provide a copy of the contact to their customer service/service department. The phone numbers (during and after hours) of the department shall be provided, in writing, to the Contracting Officer and Engineering POC no later than fifteen (15) days prior to the contract start date. The personnel of the department shall be able to understand, read, fluently speak, and legibly write the English language. Contractor shall provide their employees with appropriate protective clothing, equipment, tools and apparel as prescribed by the Occupational Safety & Health Administration (OSHA). The technician shall be dressed in appropriate uniform and wear an identification badge at all times when servicing at the VA facility. REPORTS The Contractor must provide reports of all maintenance/service/repairs done within 48 hours after completion of maintenance. The Contractor must outline on the report all SOW items worked on listed on part 4. This report shall be provided to the engineering POC as listed in the contract or the assigned designee. ADMINISTRATION Facility Orientation: An initial orientation of the facilities will be conducted by the POC at the start of the contract. The contractor shall be responsible for conducting any orientation for new employees thereafter. Accident Reporting: In the event an accident occurs on the Department of Veterans Affairs property or involving Government personnel or property, the contractor shall contact the VA Police immediately. A report shall be provided to the Contracting Officer and POC in writing that shall include the following: (1) the time and date of occurrence; (2) the place of occurrence; (3) a list of personnel directly involved; and (4) a narrative or description of the accident to include a chronological order timeline of the accident and circumstances; (5) corrective action to prevent future occurrences.
