- THIS SPECIAL NOTICE IS NOT A REQUEST FOR QUOTES. This notice is for informational purposes only. This Sources Sought Notice 36C24421Q0036; is neither a solicitation nor a promise of future work. Philadelphia VA Medical Center, 3900 Woodland Ave, Philadelphia, PA 19104-4551, has a potential requirement for a SkyPlate Detector. Small Business, SDVOSB, and/or VOSB manufactures and authorized distributors are encouraged to submit your company s information and capability statement in response to this Sources Sought Notice. The Department of Veterans Affairs (VA), VISN 4, Network Contracting Office 4 (NCO 4), is seeking information and potential qualified sources capable of meeting the Statement of Work (SOW) for a Brand Name SkyPlate Detector, for the Philadelphia VA Medical Center (VAMC), Philadelphia, PA. The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of interested parties to demonstrate to the government that they are capable of providing the equipment or supply that fulfills the required minimum specifications mentioned above. Responses to this Sources Sought should include company name, address, point of contact, phone number, point of contact e-mail, DUNS Number, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) 334516, (size standard of 1000 employees). Respondents shall also answer the following questions: Size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), HUBZone, Woman Owned Small Business (WOSB), Small Business (SB), Large Business, etc. Is your company considered small under the NAICS code identified under this Source Sought? Is your company the manufacturer, distributor, or equivalent source for the type of items required with the characteristics referenced above? If you accompany is a large business, do you have any authorized distributors? If so, please provide the name(s), telephone Number(s), Point(s) of Contact and size status of the businesses authorized to distribute your products. If your company is a small business and an authorized distributor/reseller for the types of items required with the characteristics referenced above, do you alter; assemble; or modify; the items requested in any way? If your company alters the items, state how and what is altered; assembled; and/or modified? Does your company have an FSS contract with GSA or the VA NAC or a GWAC contract holder with any other federal agency? If so, please provide the contract number. If your company holds a FSS GSA/VA NAC contract other federal GWAC contract holder, are the items you are providing information on available on your schedule/contract? General pricing of your products is encouraged. Pricing will be used for the purpose of market research only. It will not be used to evaluate for any type of award. Statement of Need for: BRAND NAME: Philips SkyPlate Detector (Replacement) Introduction The Department of Veterans Affairs, Philadelphia VA Medical Center, has a Brand Name or Equal Requirement for a SkyPlate Detector. BACKGROUND: CMC VAMC Philadelphia Diagnostic Radiology Department has a bona fide requirement to procure a replacement for a Philip SkyPlate Detector that will be in conjunction with the hospital s portable x-ray machine. The current detector is considered inoperable due to improper handling by the hospital s staff. The Skyplate Detector is an imaging receptor that is key for the functioning of the facility s Philips Healthcare X-Ray machine. This equipment part is mandatory, because the portable x-ray machine cannot take an image without it in place. The equipment is designed to provide high-resolution imaging, so that x-rays can be taken that achieve Detective Quantum Efficiency (DQE). SCOPE OF WORK: This SOW covers the following activities and deliverables which includes the manufacture and delivery of one replacement Skyplate Detector to be used with the department s portable X-Ray scanner. The Contractor agrees to perform all services with the standard of care, skill, and diligence normally provided by a professional organization in the performance of similar duties. LINE ITEM PART NO. DESCRIPTION/SUPPLIES/SERVCES 0001 4512-134-72142 SKYPLATE DETECTOR (REPLACEMENT) Salient Features (Brand Name): Philips Healthcare SkyPlate Part No. 4512-134-72142 The dimensions of the SkyPlate must be size Large, 35 cm x 43 cm or 14"" x 17 . Ability to transmit images remotely to the Philips iSite/Intellispace system. Lightweight and designed for high-resolution imaging. Must be new. Must include rechargeable battery. SkyPlate must be in conjunction with our Philips Mobile Diagnostic Wide Dynamic Range machines. Manufacture Warranty Manufacture Distribution Letter Training and Installation: Training and installation not required by Vendor. Quantity - 1 Delivery: All items must be FOB Destination, 30 Days ARO, to the following location: Philadelphia VA Medical Center 3900 Woodland Avenue Philadelphia, PA 19104 Monday Friday from 8:00am 4:00pm This is not a request for quote or proposals. The intent of this Sources Sought Notice is to identify potential Small Businesses especially any SDVOSBs capable of providing the brand name or equal equipment. Information provided by potential sources will be utilized by the Government to determine market availability and procurement strategy. An award of a contract will not result from this Sources Sought Notice. Vendors who can meet the requirements listed above should provide the following information: Business Name Business Address Business Point of Contact, including phone number and email address Business size Business Socioeconomic Status Federal Supply Scheduling (FSS) number, if the items required by the Government are available for purchase on an FSS contract. Any other pertinent capability information that relates to the above stated requirement.
