- North OKC VA Outpatient Clinic Lease Statement of Work Due to new initiatives and growing Veteran population in the region, The Department of Veterans Affairs is seeking a commercial lease for clinical / medical use in the Northern Oklahoma City / Edmond area. The lease space shall yield approximately 22,000 contiguous net usable square feet of medical office and related space. The rentable space shall be available for use by tenancy for personnel, customers, furnishings, and equipment. Net usable square feet does not include such areas as stairs, elevators, mechanical/utility rooms, shafts, chase, public corridors, and public restrooms required by local code. The space must be handicap and public transportation accessible, ADA compliant, and can be provided by existing medical office space or modification of existing space. The building and parking area must be fully accessible, meeting all requirements of the VA Barrier Free Design Standard (PG-18-13, http://www.cfm.va.gov/til/dGuide/dgBarrFree.pdf). Clinic floorplan and layout will be in compliance with the VA Patient Aligned Care Team (PACT) Space Module, which is found at https://www.cfm.va.gov/til/dGuide/dgPACT.pdf. Layout will promote functional flexibility to end users. Lessor is to provide both interior and exterior signage. Exterior sign to be in accordance with Sign Type EI-03.01 (Page 4-6-15) Message F within the Exterior Signage Design Guide located on the TIL (https://www.cfm.va.gov/til/signs/Signage04-Exterior.pdf). Interior signage to be designed and installed in accordance with the Interior Signage Design Guide located in the TIL. Final approval will be by VA Project Team. Telecommunications shall be provided and installed by the Lessor as part of the space buildout. Design shall follow the Office of Information & Technology Design Guide (CBOC Inside Plant Information Transport Systems Specificatiions). Lessor s A/E shall identify workstation locations as well as locations for data and power needed. All disciplines shall be coordinated with VA Project Team for confirmation. The lease term requirement is for five (5) years firm and with five (5) 1-year options, with the Government having termination rights after ten (10) years giving 180 days written notice to the lessor at anytime on or after the fifth year. The new clinic will accommodate the following services, see attached space plan for room allotments: Primary Care Mental Health Tele-Mental Health Dental Administrative The delineated area is for the proposed locations is as follows: North: Northwest 164th Street South: West Britton Road East: North Pennsylvania Avenue West: North MacArthur Boulevard On-site parking with a minimum total of ninety (90) individual spaces reserved for Government use, including required handicap accessible spaces in compliance with local and VA code, whichever is more stringent. All services, supplies, utilities, partitioning, and required alterations are to be provided as part of the rental consideration. Full Service lease (list is not exhaustive): Heat Electricity Power Water (hot & cold) Snow removal Trash removal Chilled drinking water Air conditioning Toilet supplies Janitorial service & supplies Full or Part Day Service depending on the size and daily traffic of the clinic Elevator service (as applicable) Window washing On-site parking spaces (minimum 120 spaces, minimum 2 parking spaces designated van- accessible/handicapped ADA-compliant parking or minimum number of spaces required by local code or VA standard, whichever is more stringent) Initial and replacement lamps, tubes, and ballasts Pest control Preventive pest control Same day response to reports of the presence of pests Lawn and landscaping maintenance Lease will be a 5-year fixed firm term lease with five 1-year option terms New or older, very well maintained Handicapped accessible; shall meet the VA Barrier Free Design Manual Preferred single floor occupancy Building security to ensure safety of sensitive business documents Provides VA with ability to secure offices/clinic space; e.g. key, ID codes VA specified directional signage, internal and external No ACM (no asbestos containing materials) ARCHITECT -ENGINEER SCOPE OF WORK - GENERAL: The Architect-Engineer (A/E) firm is to provide services for investigation, preparation of working drawings, construction documents, construction period services, and cost estimates for lease design of a new Outpatient Clinic in Bartlesville Oklahoma. Work includes, but is not limited to, interior demolition, tenant improvement, some interior construction, electrical, HVAC modifications, interior design considerations, furniture design/coordination with separate vendors and coordination of design efforts with the building owner. Design will be required to follow State and Local building codes and regulations. Design will also incorporate all Joint Commission requirements, and, as such, the Facility Guidelines Institute s Guidelines for Design and Construction of Hospitals and Outpatient Facilities will be used as the primary standard for design and construction of Leased Facilities. Provide As-Built Record drawings of As-Constructed conditions in the AutoCAD 2017 format or as approved by the Contracting Officer, or designated Government Representative. DRAWINGS: All drawings shall be prepared in latest version of AutoCAD using the VHA National CAD Standard Application Guide. Use 22 x34 paper with minimum 1/8 text. Size and text will allow printing of readable half size drawings on 11 x17 paper. Drawings will consist of a title sheet followed by the applicable architectural/engineering order of drawings. A/E shall provide a separate file for each drawing. Multiple drawings as individual paper spaces in a single DWG file are not acceptable. Drawing shall include a title page with signature blocks for the following: Medical Center Director Associate Medical Center Director Chief of Staff Associate Director Patient Care Services Chief Engineering Service Safety Manager Industrial Hygienist Infection Control Nurse Energy Manager Blank Signature Block Blank Signature Block Blank Signature Block Blank Signature Block Blank Signature Block Each drawing shall have a legend as well as follow the VA s CAD standards located on the TIL (https://www.cfm.va.gov/til/projReq.asp.
