Page 2 of 2 REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 8, is seeking information and sources that can provide Community Based Outpatient Clinics (CBOCs) to serve its Veterans population in the United States Virgin Island s (U.S. Virgin Islands) of St. Thomas and St. John. The Authority for this procurement is VA s health care resources sharing authority in Title 38 United States Code (U.S.C.) Sections 8153. Place of Performance: The clinic location must be centrally located in the U.S. Virgin Island of St. Thomas, with easy access to public transportation. The location should provide adequate parking space and access to persons with disabilities. Period of Performance: The anticipated period of performance for this contract will be from August 09, 2021 through August 08, 2026 for the base year and four (4) One (1) year option periods. Qualifications: The contractor shall provide primary care CBOC services, both medical (physically on-site) and mental health care (either physically on-site, by telehealth, or by referral) and offer support services such as pharmacy, laboratory, and x-ray. The contractor shall also provide primary care CBOC home-based primary care (HBPC) and home telehealth to meet the primary care and mental health needs of Veterans who have difficulty accessing clinic-based care. Access to specialty care and Integrative Health is not provided on site but may be available through referral or telehealth. The contractor shall provide Patient Aligned Care Team (PACT) staffing in numbers and qualification capable of fulfilling the standards modeled after Patient Centered Management Module (PCMM) (formerly known as Primary Care Medical Home). The contractor shall provide a sufficient number of primary care providers so that each primary care provider has a caseload ratio that meets the demand to meet VA standards. The current standards are 1200 active patients per full time physician and 900 active patients per full time midlevel provider. The contractor may use as reference the VA guidelines in accordance with PCMM Handbook 1101. The contractor must establish and implement contingency plans for ensuring patients receive continuity of and access to appropriate primary care during periods of inadequate resources, extended staff absences, staff turnover, understaffing, and nature-related events (e.g., extreme weather conditions, natural disasters). If the number of patients reaches 90% of maximum panel size assigned by the facility the contractor shall communicate to the VA, the contractor s future staffing plan to ensure staffing ratio standards remain in accordance with PCMM staffing standards. This is a request for information and sources only, which may or may not lead to a future solicitation. This is not a request for proposal (RFP).
