- VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-07 effective 08-31-2020. This solicitation is set-aside for: SDVOSB The associated North American Industrial Classification System (NAICS) code for this procurement is 221310, with a small business size standard of $30 million. The FSC/PSC is H346. The Department of Veterans Affairs Network Contracting Office 8-San Juan, Puerto Rico is seeking a qualified technician for the repair of the maintenance and repair of the facility potable water chlorination skids. All interested companies shall provide quotations for the following: See attachment B Price Cost Schedule, See attachment C Clauses & Provision, See attachment D Wage Determination. SCOPE OF WORK FOR THE MAINTENANCE AND REPAIR OF CHLORINATION SKIDS AT THE VA CARIBBEAN HEALTHCARE SYSTEM OBJECTIVE: The task covered by this statement of work is to provide qualified technician for the repair of the maintenance and repair of the facility potable water chlorination skids. REQUIRED SERVICES: Qualified technician for the repair, calibration, and adjustment of the following chlorination skids: South Bed Tower potable cold water located in building basement pump mechanical room. South Bed Tower potable hot water located in building penthouse mechanical room. Building 1 potable hot water located in sub-basement mechanical room. Building 1 potable cold water located in sub-basement elev-80. CLC 1 potable cold water located in CLC mechanical pump room. CLC 1 potable hot water located in CLC mechanical pump room. Potable water reservoir tank 1 located in chlorination mechanical room next to the tank. Potable water reservoir tank 2 located in chlorination mechanical room next to the tank. Preventive maintenance requirement. Weekly: Check tubing connections, gaskets and unions for leaks. Repair if necessary. Check operational parameters and adjust if needed. Check sampling filter media and replace if needed. Check pump connectivity with controller and establish connection if needed. Check pump operation and replace if needed. Check brine tank salt level and add if necessary. Check water softeners operation, regenerate resin if needed or replace if necessary. Monthly: Check Envirox Generator cassette and replace if necessary. Check Sodium Chloride 25% level and refill if necessary. Check Sodium Hypochlorite Solution and refill if necessary. Check PH sensor and adjust or calibrate if necessary. Check pressure gauges and replace if necessary. Check secondary containment. Maintain secondary containment clean. Check pressure regulator and replace if necessary. Check Chlorine Cl2 sensor and adjust or calibrate if necessary. Provide monthly report with findings and corrective actions as well as operational parameters and control limits. Provide recommendations, included in the monthly report, for system improvement or efficiency. GENERAL REQUIREMENTS: Contractor must provide a certified water treatment operator class 4 to provide service in the mentioned units. Contractor must have at least 10 years of experience in the water treatment industry. ITEMS AND DATA TO BE FURNISHED BY THE GOVERNMENT: The VACHS Manager or his designee will coordinate site access and site layout to include any equipment storage requirements. The following solicitation provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items. FAR 52.212-3, Offerors Representations and Certifications Commercial Items www.sam.gov registration is required to be complete at time of submission of response. Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. The following contract clauses apply to this acquisition: FAR 52.212-4, Contract Terms and Conditions Commercial Items (OCT 2018) FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (JUN 2020) ADDENDA 2 to 52.212-2 (a) Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items. The following subparagraphs of FAR 52.212-5 are applicable: All quoters shall submit the following: Contractor must provide a certified water treatment operator class 4 to provide service in the mentioned units. Contractor must have at least 10 years of experience in the water treatment industry. All quotes shall be sent to the karina.miranda@va.gov Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. Base plus Four Option Years. The following are the decision factors: This requirement will be awarded to a responsible contractor whose proposal represents the best value to the Government, considering price and technical factors, with technical and past performance factors more important that price. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. Technical Capability: Provide technical performance plan and proof of applicable certifications, license, and insurance. Past Performance: Prospective contractors may provide a list of five business references for which the contractor performed the same or similar scope as required in the statement of work. The reference information shall include name of business; name and title of point of contact; brief description of services performed; and date services performed. Contractor s Proposed Rate. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Quoters shall list exception(s) and rationale for the exception(s), if any.
