- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is W911YNY-20-R-0005 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses. The associated North American Industrial Classification System (NAICS) code for this procurement is 621999 with a small business size standard of $16,500,000.00. This requirement is a Small Business 8(a) competitive Set-Aside and only qualified offerors may submit bids. See attached solicitation and exhibit A, B &C. Questions received and Answers given are also attached. Answers to questions are to be relied upon by prospective offerors. Bid MUST be good for 30 calendar days after close. This solicitation requires registration with the System for Award Management (SAM) in order to be considered for award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Registration must be ""ACTIVE"" at the time of award. In addition to this solicitation see attached �52.204-24 and 52.204-25. Requirement. The Florida Army National Guard has a requirement for behavioral health professional services (including case management), medical, and dental non- clinical case management and administrative care coordinator support services for an average of 700 duty limiting conditions requiring a profile per month (as described in AR 40-501, AR 40-502 and DA PAM 40-502), a continual monthly average of 500 in depth cases (expected to take more than 30 days to resolve) requiring additional paperwork and board actions with the goal of maintaining a medically deployable force. The case management team is expected to track duty limiting behavioral, medical, and dental issues by Soldier. Duty limiting condition information is expected to be synchronized for Soldiers with multiple issues when clinically acceptable between behavioral and medical personnel to streamline contact with units and Soldiers to meet readiness requirements. The contractor shall ensure annual medical/dental requirements are being maintained to less than 400 service members in a non-ready (MRC 4) status due to lack of annual screenings.
