Description
PART 13 BLANKET PURCHASE AGREEMENT DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK: The Minneapolis Veterans Affairs Healthcare System (MVAHCS) has a continued need for the uniform conveyor and scrub machines. The current hardware and software continues to fail and needs to be upgraded. All the equipment and machines are in good working condition but the problem remains with the outdated hardware and software. Our current White Conveyor equipment can only be run with their software and request this go Sole Source. REQUIREMENTS: White Conveyor Inc is to provide PC s, a server, and all ancillary components for this requirement. In addition, MVAHCS will need the PC hardware all set up and training provided on how to use the new software while on site installing. Additional equipment and services include: Power supply back up Dell computer for UPI Dell 17 monitor for UPI External hard drive 1TB OPC software, ethernet KEP All in one PC, ASUS Workstation PC w/monitor SQL Server PC IDENTIFICATION, CHECK-IN, PARKING, DRESS, AND SMOKING REGULATIONS: Each contractor to perform work within the VA facilities must acquire a badge at the security desk upon entry into the facility. The contractor s staff will be required to check-in with the Laundry office before beginning work each day. There, they will receive keys to gain access to permitted areas and then return them after every pickup. The vendor s employees shall wear visible VA contractor identification at all times while on VA premises. Dress shall consist of uniforms, khakis or similar clothing along with work shoes. Jeans are not allowed. Smoking is prohibited inside any buildings at the VA. Possession of weapons or contraband is prohibited and shall subject contractor employee to arrest and termination from future performance under this contract. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. GENERAL/ ADDITIONAL REQUIREMENTS No BAA required. Contractor will be on site a total of 4 days completing the installation. The following security requirements are to be included: VA directive 6500.6, Appendix C, paragraph 2.a, 3.a, 3.f, 6.a, 6.b, 6.c END DESCRIPTION/SPECIFICATIONS/ STATEMENT OF WORK
