- FEDERAL EMERGENCY MANAGEMENT AGENCY (FEMA) Occupational Medical Services (DR-4559-LA) REQUEST FOR INFORMATION (RFI) 70FBR620Q00000062 � THIS REQUEST FOR INFORMATION (RFI) is issued in accordance with FAR 15.201(d) & (e). This RFI does not constitute a Request for Proposal (RFP), Invitation for Bid (IFB), or Request for Quotation (RFQ), and it is not to be construed as a commitment by the Government to enter into a contract, nor will the Government pay for the information submitted in response to this request. All information contained in this RFI is preliminary and is subject to change if, and when, a solicitation is issued. Responses to this notice are not offers and cannot be accepted by the U.S. Government to form a binding contract. In accordance with the FAR, the Government may be issue sole source contracts. If a competitive solicitation is released, it will be synopsized on the Beta.Sam.gov website. It is the responsibility of the interested parties to monitor this site for additional information pertaining to this RFI, or future solicitations. � BACKGROUND The Federal Emergency Management Agency (FEMA) is seeking non-personal services clinic medical services in support of disaster operations for DR-4559-LA for two (2) primary basecamps slated to be established in Lake Charles, Louisiana, in Calcasieu Parish or other locations throughout Louisiana as required by the Government. PROJECT SCOPE/DELIVERABLES The Contractor shall provide Occupational Health Services to include the care for work-related and non-work-related illnesses and injuries, respond to medical emergencies, immunizations, over the counter medication; and a Medical Surveillance Program, to include nursing assessments utilizing nurse protocols, preventative and routine health screenings, health risk appraisals, health education programs and health units. The health unit support shall require the Contractor to establish and maintain the health stations at a designated government locations in Louisiana, as directed by the Government.�� Medical services provided under this contract are limited to FEMA employees, with the exception of emergency situations that require immediate life-saving care. Employees will be required to show their identification badge in order to obtain services, however no information from the identification badge will be retained by the contractor. These services allow FEMA employees to receive basic medical care such as: ��������� Immunizations (e.g. Flu shot) ��������� Assistance with administering of prescribed medications (e.g. insulin shot) ��������� Provision of over-the counter-medications or products (e.g. Aspirin, bandages) ��������� Using temporary immobilization devices while transporting an accident victim ��������� Cleaning, flushing, or soaking wounds on the surface of the skin The period of performance may be three (3) months with monthly option periods. SUBMISSION OF CAPABILITY PACKAGES INSTRUCTIONS Interested vendors shall submit a brief capabilities' statement package via email to the Points of Contact listed below. There is no page limitation on your submission. Respondents shall also provide a cover page with their Company Name, DUNS Number, CAGE Code, Company web address, Company Address, Point of Contact, Telephone Number, e-mail Address, the Size of your Organization, and, if applicable, the small business socioeconomic category. This documentation should address, at a minimum, the following to demonstrate the vendor's capability to perform the requirements. Vendors are requested to respond to one, some, or all components. (1) The vendor's technical capability to deliver the aforementioned services;�� (2) Past experience with similar requirements along with the contract type/pricing methodology; and (3) The vendor's capacity in the geographic location (4) The vendor's ability to manage, as prime, the types and magnitude of services required. Please provide additional information you deem relevant to respond to the specifics of the RFI. The Government is not requesting any proprietary information to be submitted, nor shall it be liable for any consequential damages for proprietary information included. Any information submitted to the RFI (e.g. capability statement) is subject to disclosure under the Freedom of Information Act, 5 USC 552 (a). All submissions submitted will not be returned. RESPONSE DEADLINE: RFI responses and comments are due by September 17, 2020 at 2:00 pm (EST) via email to the POC listed below.
