This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. �The solicitation number is 246-20-Q-0206 for Digital Mobile Radiographic System to include installation and warranty.� This solicitation is issued as a Request for Quotation (RFQ).� The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-07. �This acquisition is 100% Set aside for Small Business.� The NAIC code is 423450 and the small business size standard is 200 employees. The closing date for receipt of quotes is September 15, 2020 at 4:00 pm.� Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:00 p.m., on September 15, 2020.� The offer must be submitted by email: showing the solicitation number, and your name and address.� Qualified vendors will review the following and submit applicable information.� No phone inquiries! Technical Specification:� See attached Statement of work Evaluation Criteria: The due date for this requirement is September 15, 2020 by 4:00 PM CT.� THE INDIAN HEALTH SERVICE WILL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO RESPOND TO THESE EVALUATION CRITERIA AS FOLLOWS: ���� A. Registration in the System for Award Management (SAM) database (reference https://www.sam.gov/portal/public/SAM/.� Please note that all parties interested in doing business with the Indian Health Services must be registered in the SAM database. � �����B. Firm-Fixed price quote. �����C.Technical specifications (section III) and submittals of the Statement of Work. Proposals shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of �B� and �C� (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows: Quotes shall be ranked by price, from low to high. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote.
