- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR (Federal Acquisition Regulations) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is Request for Quote-20-172. The solicitation document incorporated provisions and clauses are those in effect through FAC (Federal Acquisition Circular) 2020-05. This is a Small Business Set Aside, Brand Name or Approved Equal solicitation. NAICS code is 334510 � Manufacturing Electromedical and Electrotherapeutic Apparatus Manufacturing. Small Business Size Standard is 1250 Employees; The unit pricing must be all inclusive (to include but not be limited to travel, lodging, per diem, fringe benefits, federal, state, and local taxes) plus all other costs pertinent to the performance of this contract. Utilize your most competitive and reasonable rates. Please see attached SF18 for Delivery Information along with attached Specifications;��������������������������������� The Great Plains Area Office is soliciting for a Firm Fixed-Price, Brand Name or Approved Equal Quote for the purchase of: Zeiss OS Cirrus HD-OCT (Model 6000) with Angio Plex OCT Angiography, Version 9.5. Includes GCGPA, Smart HD Scans, FastTrac, Adv RPE, GCA, Ant Seg Basic; To be included in package is Anterior Segment Premier Module for the Woodrow Wilson Keeble Memorial Health Care Center, Sisseton IHS, in Sisseton, South Dakota; FAR 52.212-1 - INSTRUCTION TO QUOTERS - Commercial Items (March 2020) see attached clauses for full text;�� Quote must contain the following documents in order to be considered responsive and eligible for an award: 1. Complete and sign the attached SF18; 2. Complete the attached FAR provisions and clauses applicable to this acquisition, including: FAR 52.212-2 � Evaluation � Commercial Items (Oct 2014). The Government will award a contract resulting from this solicitation to the lowest responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be sed to evaluate offers: Ability to meet or exceed the requirements/specifications. Price Evaluation - The Government shall conduct a price evaluation of all technically acceptable offers.
