Description
PURPOSE OF AMENDMENT 1 IS TO UPDATE THE PERFORMANCE WORK STATEMENT (PWS) TO REFLECT ANSWERS TO RFI 1.� �ALL OTHER TERMS AND CONDITIONS REMAIN THE SAME. This is a combination synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; proposals are being requested and a separate, written solicitation will not be issued. ALL PROPOSALS MUST BE SUBMITTED VIA EMAIL TO THE PRIMARY POC LISTED ON THIS POSTING. The 187th Fighter Wing (FW), Alabama Air National Guard, has a requirement for an Optimizing Human Weapon System Services (OHWS) program at the 187th Fighter Wing, Dannelly Field, Montgomery, AL, that is to reduce the frequency and severity of neck and back pain injuries, reduce duty time lost, improve quality of life, enhance readiness, and support fighter aircrew retention. This program is described as pre-habilitation. Pre-habilitation involves prevention of disability and pain experienced as a result of fighter pilot operations through a program specifically designed to train and otherwise prepare aircrew for the rigorous demands of fighter aircraft. This is a non-personal healthcare services contract, as defined in FAR 37.101. This requirement is a 100% Small Business set-aside requirement. The North American Industry Classification System (NAICS) Code is 621340 and the Size Standard for Small Business is $8.0M. The Service Contract Award Wage Determination 2015-4607 Revision 10, dated 18 June 2020, is applicable for this requirement.� Requests for Information (RFI) shall be sent via email to: TSgt Ben Scott, benjamin.scott.18@us.af.mil. Subject title must include the title of this solicitation.� Offerors shall utilize the Past Performance Questionnaire to identify respondents IAW instructions in the Combo. Complete required blocks in Sections 1 and 2 only.� The Government intends to issue a Firm Fixed-Price contract for these services, see Evaluation and Award Factors, Paragraph 4, for award basis. Please review these documents in their entirety for a complete description of this requirement and required submissions for your proposal to be considered technically acceptable. All required attachments, documents, and information stated in the Instructions to Offerors must be included.�The Government reserves the right to cancel this solicitation, either before or after the response date. Proposals shall be valid until 30 October 2020. PARTIAL OFFERS WILL NOT BE CONSIDERED. PARTIAL AWARDS WILL NOT BE MADE.
