- This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6 and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is 36C24519Q0564 and is issued as a Request For Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2019-01, fiscal year 2019. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This requirement will use the cascading set-aside strategy, compliant with 38 U.S.C. 8127 (AKA Kingdomware). The evaluation of offers received in response to this solicitation will use a tiered or cascading order of precedence. ""Tiered evaluation of offers."" also known as ""cascading evaluation of offers,"" is a procedure used in negotiated acquisitions when market research is inconclusive for justifying limiting competition to small business concerns or sub-categories of small business concerns. Offers from both small and other than small business concerns will be evaluated in the following tier order: Service-Disabled Veteran-owned Small Business (SDVOSB), Veteran-owned Small Business (VOSB), all other small business concerns, other than small business. The North American Industry Classification System (NAICS) code is 621511, and the size standard is $32.5 Million. (v) This requirement consists of one (1) line items: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT 0001 Automated Immunochemical Fecal Occult Blood Testing Base with 4 one-year options EA (vi) The method of iFOBT must be completely automated. To be acceptable, the proposal shall meet all requirements listed below in the Statement of Work and the price must be reasonable. STATEMENT OF WORK AUTOMATED IMMUNOCHEMICAL FECAL OCCULT BLOOD TESTING (iFOBT) Veterans Integrated Service Network (VISN) 5 LABORATORIES A cost-per-reportable-result Blanket Purchase Agreement (BPA) is being sought for automated immunochemical fecal occult blood testing (iFOBT). This BPA covers all sites in VISN 5, i.e. Baltimore, MD; Martinsburg, WV; Clarksburg, WV; Beckley, WV; Huntington, WV and Washington DC. Baltimore, MD - 10 N Greene Street, Baltimore Md 21201 Washington, DC 50 Irving Street NW, Washington DC 20422 Martinsburg, WV 510 Butler Avenue, Martinsburg WV 25405 Clarksburg, WV 1 Medical Center Drive, Clarksburg WV 26301 Beckley, WV 200 Veterans Avenue, Beckley WV 25801 Huntington, WV 1540 Spring Valley Drive, Huntington WV 25704 DESCRIPTION OF SERVICES / REQUIREMENTS VISN 5 is establishing this open market contract for cost-per-reportable-result supplies for automated immunochemical fecal occult blood testing (iFOBT) and services rendered for the analyzer as set forth below: The method for iFOBT must be completely automated. The vendor will supply the most current analyzer instrument and cartridges for testing to each medical center in Veterans Integrated Service Network (VISN) 5. The VA will pay on a cost-per-reportable-result basis for this service. The vendor will maintain all maintenance on the analyzer, including upgrades and updates to hardware and software at no cost to the government. Normal repair service shall be available 24 hours per day, 7 days per week, exclusive of federal holidays at no charge to the government. Federal holidays include: New Year s Day, Martin Luther King Day, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day. Emergent repair service shall be available 24 hours per day, 7 days per week, inclusive of federal holidays at no charge to the government. Federal holidays include: New Year s Day, Martin Luther King Day, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day. Preventative maintenance shall be performed at least twice every year of the contract term. All reagents, controls, standard supplies, consumable/disposable items, parts, accessories, equipment and any other item required for the proper operation of the vendor owned iFOBT testing equipment shall be provided by the vendor. Equipment must be the latest state of the art technology. Equipment must have accurate metering capability for monitoring reagent usage and accept the VA s monthly Laboratory Management Index Program (LMIP) statistics generated from Veterans Information System and Technology Architecture (VistA). The iFOBT analyzer instrument shall have the capability to interface with the VA Vista computer system. Test results shall be electronically sent to Vista. TERM OF CONTRACT This contract is effective as follows: Base: October 1, 2019 September 30, 2020 Option 1: October 1, 2020 September 30, 2021 Option 2: October 1, 2021 September 30, 2022 Option 3: October 1, 2022 September 30, 2023 Option 4; October 1, 2023 September 30, 2024 The contract is subject to the availability of funds. (vii) All material shall be delivered to each VA Medical Center listed above. (viii) 52.212-1 Instructions to Offerors - commercial Items, applies to this acquisition. There is no addenda to the provision. (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price and meeting or exceeding Statement of Work requirements. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- the offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via the System for Award Management (SAM) Website through http://www.sam.gov/portal. If the offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (p) of this provision. (xi) 52.212-4 Contract Terms and Conditions - Commercial Items: applies to this acquisition.
