- Statement of Work (SOW) Background: The New Jersey VA Healthcare System, East Orange Campus is currently seeking preventative maintenance service for their Steris sterile processing equipment. The contractor shall perform the PM in accordance with the original equipment manufacturers (OEM) specifications. The contractor is required to furnish all parts, supplies and labor to perform unscheduled repairs when requested unless the service request is subject to the vendor s parts exclusion policy. The PM shall include cleaning, calibrating, adjusting, specifications and lubrication. The contractor shall replace all worn or defective part. The contractor shall verify manufacturer s operational characteristics. Replacement parts shall be OEM manufactured parts or OEM authorized/approved equal. In some cases, refurbished items may be approved by the COR if new or OEM parts or equivalent parts are not available. Upon completion of the PM, the equipment shall be returned to full functioning condition. The Contracting Officer Representative (COR) or authorized VA Point of Contract shall inspect the completed work prior to the technician leaving the facility. This inspection will ensure full functionality and will be used as justification for certifying payment invoices. The service representative shall provide a service report upon completion of the PM or repair. The report shall contain at minimum the following information: A brief description of the purpose of the call Work performed Parts used and time spent for labor and travel Any outstanding deficiencies Time and date of the call Name of technician who performed the work Contractor shall respond to requests for technical telephone support within 2 hours of receipt of the telephone request. Contractor shall be on site to begin repairs within eight (8) business hours of initiation of service call by the Medical Center. Repairs shall be completed, and unit fully returned to manufacturer specifications within sixteen (16) business hours of initiation of service call to the maximum extent practicable. Major assemblies replaced shall be guaranteed by contractor against defects and/or failure for a period of one (1) year from date of installation. All other components shall be guaranteed for a period of ninety (90) days. Service representatives are required to supply the Medical Center with Material Safety Data Sheets (MSDS's) on any chemicals that are brought into the Medical Center prior to usage. Contractor shall adhere to all federal, state, local and VA policies and procedures during performance. The Contracting Officer or his/her designee will notify the contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work and hold the Contractor in default. Period of Performance: The estimated base period of performance for this requirement is 10/1/2020 through 9/30/21 with the provision of four (4) one-year options. Place of Performance: East Orange VA Medical Center 385 Tremont Avenue East Orange, New Jersey 07018 Hours of Operation: All work shall be completed during the hours of 08:00 to 20:00, Monday through Friday. This excludes any off hours work as agreed to by the Government and the Contractor on a case by case basis.
