- St. Cloud VA Health Care System Industrial Hygienist (IH) Abatement Oversight for Building 29 Expand Optometry Project Vendor to provide services of an Industrial Hygienist (IH) to monitor asbestos abatement at VA Medical Center, 4801 Veterans Drive, St. Cloud, MN. The asbestos abatement will take place in various rooms located in Building 29 on the VHASTC campus. Overview of service includes: A licensed asbestos abatement contractor will be removing asbestos containing floor tile and mastic, pipe insulation, lead-based paint on doors, frames, windows and lead-glazed ceramic wall tile. For estimating purposes, IH services will be performed during normal business hours Monday through Friday, 8:00 am to 4:30 pm minus Federal Holidays. Some work may be required to continue after 4:30 pm. Due to potential interruption of normal VA clinic operations, work may have to be completed over multiple weekends, beginning at 4:30 pm Friday and completed by Sunday. Tentative abatement schedule in which IH oversight is required is as follows: Abate floor and wall tile and remove asbestos-containing pipe insulation in Room 22 (larger purple highlight area on color-coded abatement plan attached below) 5 days Tuesday 10/13/2020 - Monday 10/19/2020 Abate/remove exterior window frames (marked L1 on exterior walls on color-coded abatement plan attached below) 7 days Tuesday 10/20/2020 - Wednesday 10/28/2020 Abate mastic under floor tile and carpet in Suite 5 (pink highlight area on color-coded abatement plan attached below) 5 days Monday 04/19/2021 - Friday 04/23/2021 Total estimated days of required IH Services = 17 Vendor to provide services of an Industrial Hygienist (IH) to monitor removal of asbestos containing flooring materials, pipe insulation materials, lead glazed ceramic wall tile, and lead-based paint on windows, doors and door frames. The IH shall perform continuous air monitoring, inspection and testing outside and inside regulated areas, provide onsite analysis of air samples, ensure that asbestos contractor workers are clean shaven, and conduct pre-abatement and post-abatement inspection with the VA IH. IH shall be licensed by the State of Minnesota to conduct air monitoring for asbestos abatement projects. Provide site visits as needed. The IH shall conduct pre-abatement and post-abatement inspection with the VHASTC IH. Conduct clearance monitoring. The IH shall obtain certificate of workers acknowledgement and affidavit of medical surveillance, respiratory protection, and training/accreditation of each worker that is onsite to do the job. The IH shall notify VHASTC Safety Department or Contracting Officer s Representative (COR) if problems occur. The IH shall oversee the work to ensure that contractor is abiding by MPCA, MDH, MNOSHA, EPA and OSHA rules and regulations. Provide VA with a final report which includes all applicable from the following: air monitoring methodology, certificate of visual inspection, clearance air sampling sign-in sheet, certificate of air clearance, sample location drawings, certificate of completion, laboratory analysis results, contractor s license, contractor s insurance certificate, contractor s state notification and amendments, contractor s asbestos-related work project permit, contractor s asbestos project plan, contractor s daily sign in sheets, contractors daily log, contractor s enclosure sign-in sheet, worker s certificates/licenses/medical clearance, OSHA personal air sampling record, landfill manifest, on-site monitor qualifications, air monitoring notification, and PLM bulk laboratory reports. Payment will be made upon receipt and approval of all documentation. Any deviations in work schedule will be approved by the Contracting Officer.
