- Page 2 of 2 REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, is seeking information and sources that can provide ambulance services in Sheridan WY and surrounding areas. The NAICS code is 621910- Recreational Camps and Vacation Camps (except campgrounds) and Small Business Size Standard is $16.5 million. The information submitted shall contain the company s business size status. This is a request for information and sources only, which may or may not lead to a future solicitation! This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice. Requests for copies of a solicitation shall not be honored or acknowledged. Information should be forwarded to the Contracting Officer and Program Manager. Any questions should be addressed to the Contracting Officer, in writing, at the email address provided. Because this is a request for information only, answers to questions will not be posted. See attached document: DRAFT SOW Ambulance Services DRAFT STATEMENT OF WORK GENERAL INFORMATION Title of Project: Ambulance Service 2. Scope of Work: Life support transportation services which utilize ambulances, Emergency Medical Technician Basic, ACLS- prepared RN, equipment and facilities subject to the rules of the Wyoming Department of Public Health and Environment, and the Public Health division of each and/or county in which services are provided. 3. Background: Furnish 24-hour basic and advanced life support ambulance non-emergent and emergent services for the beneficiaries of the Department of Veterans Affairs, Sheridan Wyoming. All services provided under this agreement shall be scheduled in advance with the Contractor. While patients will be located mostly within the Sheridan, Wyoming region, transportation may include other areas of the State of Wyoming, including location of Sheridan VA Outpatient Clinics. These locations include, but are limited to: Gillette, Casper, Riverton, Powell, Rock Springs, Afton, Worland and Evanston, or surrounding areas to include Billings, Montana. The VA will pay transportation costs only for eligible VA patients as determined by VA regulations. Contractor may contact the VA Patient Travel Office or Administrative Officer of the Day if there are any questions concerning eligibility. All work under this contract shall be performed in accordance with any applicable US Department or OSHA regulations, as well as applicable State transportation or OSHA regulations, Joint Commission standards and standard industry practice. 4. Performance Period: Furnish 24-hour basic and advanced life support ambulance services for the beneficiaries of the Department of Veterans Affairs, Sheridan Wyoming. B. CONTRACT AWARD MEETING No kick-off meeting required No requirements. GENERAL REQUIREMENTS The Contractor must be licensed as both basic and advanced life support ambulance services by the state of Wyoming to provide Emergency Medical Services. The Contractor must maintain licenses/certification with the state of Wyoming, throughout the life of the contract. License/certification lapses or expiration is grounds for termination of the contract or discontinuance of further ordering from the Contractor. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Furnish 24-hour basic and advanced life support ambulance services for the beneficiaries of the Department of Veterans Affairs, Sheridan Wyoming. Basic life support patient s examples: Patient requiring cardiac monitors and assessment. Patient requiring continuous IV therapy (nitro or lidocaine stable rate). Patient requiring chronic or stable ventilator support. Psychiatric patient requiring restraining devices or sedation, and monitoring. Patient with IV additives, not of a life support nature (low dose Morphine, IV Valium, Heparin). Advanced support patient examples Patient requiring IV push medications for hemodynamic support. Patient requiring IV nitro or lidocaine with frequent titration. Patient requiring acute, unstable ventilator support. GOVERNMENT RESPONSIBILITIES [If applicable] No requirements. E. CONTRACTOR EXPERIENCE REQUIREMENTS The Contractor must be licensed as both as advanced and basic life support ambulance service by the State of Wyoming to provide Emergency Medical Services. The Contractor must maintain licenses/certifications with the State of Wyoming, throughout the life of the contract. License/certification lapses or expiration is grounds for termination of the contract or discontinuance of further ordering from the Contractor. The Contractor must maintain documentation demonstrating they meet all requirements of Federal, State and County or City codes regarding operation of this type of service. Documentation shall be made available to the VA immediately upon request for the duration of the contract. F. CONFIDENTIALITY AND NONDISCLOSURE Standard mandatory language for all task orders. The Contractor shall comply with the provisions of the Privacy Act of 1974, the Health Insurance Portability and Accountability ACT (HIPAA) of 1988. As well as all applicable VHA regulations pertaining to fire, safety, and infection control, which will be provided by the Contracting Officer Representative (COR). CONTRACTOR PERSONNEL SECURITY REQUIREMENTS New contractor personnel security requirements, which facilitate the security programs for computer systems and automated information systems implemented by the Office of Security and Law Enforcement and Veterans Health Administration. No requirements.
