GENERAL INFORMATION FOLLOWED BY A TENTATIVE STATEMENT OF WORK SOURCES SOUGHT NOTICE: This is not a solicitation but rather a Request for Information (RFI/Sources Sought) for market research purposes only. Veterans Health Affairs, Network Contracting Office 4, is issuing this Sources Sought in order to identify capable firms and obtain information for planning purposes only. Veterans Health Affairs, Network Contracting Office 4, is issuing this Sources Sought in order to identify capable firms for Graphic Design Services for the Butler VAMC. Responses shall be submitted by 11:59pm (EST) September 3rd, 2020. Responses shall be submitted via email to william.schlobohm2@va.gov . All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. All small business firms that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). While SDVOSB/VOSB contractors are preferred, all capable contractors are welcome to respond to this sources sought notice for market research purposes. All respondents shall include as part of their response a brief capability statement that covers the information in the following tentative statement of work, as well as answers to the below three (3) questions. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., socioeconomic set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses. Suggested NAICS: 541430 (Graphic Design Services) Suggested PSC: T001 (Photo/Map/Print/Publication Arts/Graphics) Open to suggestions from the market as to a more proper NAICS and/or PSC, as well as any potential GSA/FSS Schedule SIN categories. In addition to the information requested above, please include answers to the below 3 questions as part of your response to this Sources Sought: 1. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services under the same or a related NAICS codes. 2. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services that are registered under the same or a related NAICS code with the Center for Veterans Enterprise (CVE); Provide the certification type (SDVOSB/VOSB), Federal Identification Number, and state of incorporation for each. 3. Do you plan on responding to a solicitation for this requirement with a Joint Venture utilizing multiple owned companies as majority or non-majority owner?  Yes ___No ___ If yes, please identify which companies are considering a Joint Venture and the ownership of each company. SAMPLE / TENTATIVE STATEMENT OF WORK (BELOW) STATEMENT OF WORK GRAPHIC DESIGN SERVICES BUTLER VETERANS AFFAIRS HEALTH CARE SYSTEM DESCRIPTION: This contract s period of performance will be a base year (12 months) plus four (4) twelve (12) month option years. The publication timeframe: November 2020-February 2021. Timeframe for a first, designed draft (once all content/photos are received) should be approximately 3-4 weeks. The contractor shall perform graphic design services for the Butler VA Health Care System s Annual Report publication in accordance with this Statement of Work. The contractor is required to provide a minimum of 3 samples of past, original design work for review prior to award to evaluate quality and overall past performance. The contractor shall furnish all labor, equipment, supervision, management, supplies, transportation and facilities necessary to perform the requirements of this contract. Services shall include pick-up and delivery of items by contractor personnel. PLACE OF PERFORMANCE: Butler VA Medical Center 353 North Duffy Road Butler, PA 16001 SCOPE: The Contractor shall provide complete services which include all labor, material, equipment, maintenance, repair, supplies, transportation, management, expertise and supervision as requested to perform this service in accordance with this Statement of Work. The services for this contract will be to provide all resources necessary to accomplish the requirements of this Statement of Work. The project purpose is to support the provision of annual updates and news to various stakeholders including employees, elected officials, service organizations and Veterans. Contractor, in support of this purpose, will provide technical guidance and graphic design deliverables for the annual report. Samples of past Annual Reports may be found online at: https://www.butler.va.gov/news/MediaCenter.asp REQUIREMENTS: The contractor shall design a high-quality, full color, full bleed Annual Report publication from provided text and images. The length of the annual report must be between (15) fifteen and twenty-five (25) pages pending on the content for the year, and the theme chosen. The contractor must provide the following in conjunction with this service: Attend initial meeting (virtual) to discuss concept and design plan Layout all pages based on suggested concept Assist with concept if requested Modify photographs as needed for color correction and cropping Purchase stock photography when needed and approved by client Make all needed revisions through approximately 3-5 versions Provide pdf proofs for each version Provide GPO forms with completed design information Preflight artwork for printing; provide art files to printer Provide a 508 compliant version for website posting Capability of transferring high resolution images and large document files quickly, for example, viable FTP capabilities. Contractor shall be able to meet all requirements within four (4) weeks of receiving all content (including photos) from the Butler VA Health Care System and in as little as 5 revisions. All content and artwork become the property of the Butler VA. PERIOD OF PERFORMANCE: The period of performance is one base year and four one-year options. SPECIAL CONTRACTOR PROVISIONS: GOVERNMENT RESPONSIBILITY: The Butler VA Health Care System will provide awarded Contractor with images and documentation that shall be used in the report Make staff reasonably available for interviews, calls, or other communication as needed to enable Contractor to effectively produce deliverables INTERFERENCE IN PERFORMANCE REQUIREMENTS: Contractor may be required to interrupt their work at any time so as not to interfere with the normal functioning of the facility; e.g. utility services, servicing of fire protection systems, and the passage of facility patients/personnel, etc. In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the government. Environmental Management Service, Safety Department, and the VA Police Department reserve the right to request the removal from the work site any supplies and equipment it deems does not meet the codes above or regulations, to include being required to the halt of any unsafe practices observed in carrying out the contracted service. This will in no way relieve the Contractor of complying with the scope of work or service provided. END OF PERFORMANCE WORK STATEMENT
