- THIS IS NOT A SOLICITATION. This is a Sources Sought Notice only and is issued to conduct market research and gain knowledge of current market capabilities. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This notice is issued solely for information and planning purposes and does not constitute a solicitation or a guarantee to issue a solicitation in the future. This notice does not commit the Government to contract for any supply or service. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Not responding to this notice does not preclude participation in any future solicitation, if issued. This announcement is based on the best information available at this time and is subject to future amendment. SERVICES REQUIRED The contractor shall provide all vehicles, transportation, insurance, personnel, and directional materials necessary to accomplish regularly scheduled courier services for and between the Department of Veterans Affairs Medical Center (VAMC), Wilmington, Delaware and the Department of Veterans Affairs Medical Center (VAMC), Philadelphia, Pennsylvania. Services shall be provided Monday through Friday, excluding Government holidays. PLACE OF PERFORMANCE, SCHEDULE, LOCATIONS Work Hours Pick up at the Department of Veterans Affairs Medical Center (VAMC), 1601 Kirkwood Highway, Wilmington, Delaware between 12:00 Noon and 12:30pm. Delivery shall be made no later than 1:30pm to the Department of Veterans Affairs Medical Center (VAMC), University and Woodland Avenues, Room 3B134, Philadelphia, Pennsylvania 19104 (phone:1-800-949-1001 x4414) Monday through Friday, excluding National Holidays. Please list pricing for route separately. Exempted holidays include the ten (10) holiday observed by the Government and include: New Year s Day President's Day Martin Luther King's Birthday Memorial Day Independence Day Labor Day, Columbus Day Veterans Day Thanksgiving Day Christmas Day Other days specifically declared by the President of the United States to be a national holiday. Insurance The Contractor shall have sufficient insurance on contractor owned vehicles and personnel. It should be noted that blood substances will be one of the items transported by the contractor. Certificate of Insurance shall be provided to Wilmington VAMC as evidence of insurance coverage. In the event of litigation/investigation of a claim, the contractor and/or the contractor s personnel shall cooperate fully with the Government authorities and designated officials in the investigation of the claim or preparation for litigation. Inclement Weather Provision The Contractor shall contact the Contracting Officer's Representative or designee when the Contractor feels that weather indicates adjustment or cancellation of scheduled pick up/deliveries. QUALIFICATIONS The successful offerer shall meet all requirements of Federal, State or City codes regarding operations of this type of service. Each offerer shall submit, with the offer, a letter in duplicate fully describing the maker, model and year of vehicles, which he/she agrees to furnish under this proposal. All Drivers must possess appropriate vehicle operator licenses. The right is reserved to thoroughly inspect and investigate the establishment, facilities, business reputation and other qualifications of any bidder and to reject any bid, irrespective of price, that shall be administratively determined lacking in any of the essentials necessary to assure acceptable standards of performance. CONTRACTOR PERSONNEL Training Contractor's employees will have access to personal protected information (full names and social security numbers) on diagnostic specimens. The contractor must furnish privacy training for its employees which must include information regarding the Health Information Portability and Accountability Act, patient privacy and confidentiality of personal and healthcare information. Information received from the VA Medical Centers is the property of the U.S. Government. The Contractor must maintain strict security measures. All information (written, oral or electronic) is confidential and falls under the requirements for the Privacy Act of 1974 and HIPPA. The contractor shall not transfer, copy, distribute or disclose said information without direct authorization of Department of Veterans Affairs personnel. Contractor shall ensure that all contractor employees directly engaged in the transport of Diagnostic Specimens under this contract receive adequate training regarding DOT C.F.R. 49 Hazardous Material Requirements and OSHA C.F.R. 29 (with particular emphasis on transport of bio-hazardous material and Standard precautions related to blood-borne pathogens). Technical Direction Contractor s employees must understand and adhere to the shipping requirements of items transported. Blood and body fluid samples must be transported in a container specifically designed for blood and body fluid transportation. Supplies and reagent are transported in small packages, boxes, or containers. Storage requirement vary depending upon the material. Mail or other Documents are transported in sealed envelopes. Contractor shall equip courier vehicles with OSHA Standard Precautions Compliance Kit. The kit is a combination personal protection kit and clean- up kit to aid in the clean-up, transportation and disposal of up to 1 full liter of infectious blood or body fluid spills. Kits are readily available at nominal cost from numerous commercial sources. GENERAL INFORMATION The contractor must furnish everything needed to perform this contract according to all its terms. The contractor shall obtain all necessary federal, state, and local licenses and/or permits required to perform this work. The contractor will supply all packaging sufficient to achieve both frozen and refrigerated temperature control from the point of pickup point the delivery destination. Specimens transported at ambient (room) temperature will be kept from excessive hot or cold temperatures during transport. The Contractor will provide all levels of active management for deliveries including tracking of deliveries, training of qualified driver(s), pro-active communication of any issues regarding delays in delivery (traffic, accidents, weather, etc.), informing VAMC personnel in the event of a spill of hazardous materials. The Contractor will provide specimen tracking to ensure that specimens are actually received by the laboratory. Please provide with offer information on past performance with barcode scanning technology, including contact names, phone numbers, items tracked, and how long this technology has been in use. All costs for this should be included in this offer. Conservation of Utilities - The contractor must make sure employees practice resource and energy conservation. The Contractor must be responsible for operating under conditions that prevent the waste of resources and energy. Routes must be arranged in the most cost-effective way possible. Routes may be combined with other clients so long as the vendor savings are passed on to the Government and the Government's pickup and delivery times are met. Positive documentation of pickup and deliveries occurred must be provided. An accounting of each shipment must be made by the courier for each run. The contractor is responsible for creating, maintaining and disposing of government required reports specifically cited in this Statement of Work. If requested by the government, the Contractor must provide the original record or a reproducible copy of any such record within five (5) working days of the request. Vehicles used in the performance of this contract shall be licensed and meet all minimum vehicle requirements as mandated by the State in which the vehicle is operated. Under no circumstances shall any person other than the driver employed by the Contractor ride in a vehicle transporting items from the Wilmington VA or handle items being transported. QUALITY ASSURANCE Methods of surveillance The number and type of blood and body fluid specimens being transported will be documented on a manifest prior to being placed into the transport container. One copy of the manifest will accompany the shipment and one copy will be retained in the Wilmington VA Medical Center. When the specimens are delivered, they will be matched to the manifest. Unsatisfactory or late shipments will be brought to the contractor's attention. Complaints and incidents of unsatisfactory or late shipments will be tracked and monitored. All shipments received will be inspected at the destination Laboratory (drop off location) for accuracy, timeliness and breakage. Performance Standards 100% of all deliveries must be to the correct location. 98% of all deliveries must occur within the timeframes specified in section 1.1.1.2. 98% of all shipments will arrive in testing condition as described in section 3.2.1 (no breakage, shipping requirement met). Performance Evaluation The contractor s performance will be evaluated at least annually to determine if contractual obligations were satisfactorily met. When an obligation is not met, a resolution will be sought with the contractor's contract manager. In the absence of immediate resolution, payment for disputed services will be withheld until a resolution mutually agreeable to all parties is achieved. SPECIAL CONTRACT REQUIREMENTS Billing Each courier run will be billed separately and invoiced monthly. Payments will be made after receipt and satisfactory inspection of materials is verified. Security Requirements The following information must be Included on Appendix C, form 6500.6: 1, 2a, b, e, 3a, all of 6, all of 7 ($37.5O per affected individual), 9 9a(3), 9b,9c Business Associate Agreement is required. The C&A requirements do not apply, and that a Security Accreditation Package is not required.
