- SCOPE OF WORK for UST Removal 660-20-107 George E. Wahlen Department of Veterans Affairs Medical Center 500 Foothill Dr. Salt Lake City, UT 84148 Background: The Salt Lake City VA Healthcare System located at 500 Foothill Drive in Salt Lake City, Utah GEMS Program has identified the need for the closure of an underground storage tank (UST). The storage tank is a 3,000-gallon fiberglass tank installed in 1988. It is located in the service yard near building 38. Scope of Work: The work covered includes furnishing all labor, equipment, materials, and transportation necessary for the proper and safe closure of UST. There is a $425 permitted fee to the SLC Fire Department that must paid for proper execution of closure. Work is identified as: Clean underground storage tank. Tank must be certified clean before it may be filled in. Closure of UST via fill in place. VA certificate holders and state representatives must be present during the fill in place procedure. Contractor is to provide notice (five days in advance) to the COR prior to anticipated closure to ensure required personnel are present. Proper environmental engineering and control procedures shall be followed at all times. Period of Performance: Contract completion time not to exceed 60 days. Contractor Requirements: The Contractor shall be responsible for performing all work and providing all materials required to achieve the work required above. At the time of the fill, all necessary personnel must be present for process to commence. The Government will not be responsible for any additional expense to the Contractor if the conditions causing those expenses could have been discovered in an on-site inspection prior to bid submittal. Contractor shall verify measurements during the scheduled pre-bid site visit and prior to performing work on the Salt Lake City VA campus. PROOF OF LICENSE AND CERTIFICATION. Persons performing the fill and certification must be state certified to fill an UST. The Contractor shall submit proof to the Contracting Officer that they or their sub-contractor is licensed in Utah prior to the start of work. PROOF OF EMPLOYEE TRAINING AND MEDICAL EXAMINATION. Prior to the start of work, the Contractor shall submit proof to the Contracting Officer that: Employees and the Competent Person, as defined in 29 CFR 1926.1101, have been trained in accordance with the appropriate state regulations and as defined in all other applicable regulations. The Competent Person will be continuously on-site during all work activities. The employees and the Competent Person have completed all medical examinations as required by the applicable federal, state, and local regulations. Under government workplace safety regulations, the Contractor is responsible for the training of his employees. The Contractor is to ensure that its employees and the Competent Person have completed all necessary Safety Training. Project Safety Requirements: The contractor shall conduct and document daily pre-job safety and hazard awareness briefings with all contractor employees and any subcontractors prior to work starting. The contractor shall immediately notify VA Police and COR of any site security breaches or unauthorized entry into active work areas. The contractor shall ensure that they do not allow the seepage of any liquids, solvents or other materials used by the contractor to perform the work into the building floors below the work area. Additionally, the contractor shall submit Safety Data Sheets (SDS) for all potentially hazardous materials to be used in performing this work to the GEMS coordinator no less than ten days prior to the planned used of such materials for the GEMS s prior approval. Regulatory Requirements: All work is to be performed in accordance with the following regulatory requirements: All work will be performed as specified herein to be in accordance with all applicable codes indicated in the Department of Veterans Affairs (VA) Design Manuals, and VA Publications (Master construction Specifications, construction Standards, etc.) and any other applicable codes. Reference for design manuals located at: http://www.cfm.va.gov/til/dGuide.asp. All OSHA, EPA, and NFPA codes must be adhered to. State requirements and laws may differ from federal regulations. All applicable state requirements must be consulted when designing the project. If the state requirements are more stringent than the federal regulations, the more stringent standard shall be used in all cases. Local requirements and laws may differ from federal and state regulations. All applicable local requirements must be consulted when designing the project. If the local requirements are more stringent than the federal and state regulations, the more stringent standard shall be used in all cases. Compliance: The C&A requirements do not apply, and a Security Accreditation Package and/or security clause is not required.
